Fluid Accumulation After Neonatal Congenital Cardiac Operation: Clinical Implications and Outcomes

Summary: This study aimed to investigate the association between fluid balance metrics and mortality and postoperative outcomes in neonatal cardiac operation. The results showed that time to first negative daily fluid balance was associated with improved postoperative outcomes, while percentage fluid overload was not. Therefore, specific treatments to achieve an early negative fluid balance may reduce postoperative care durations.

Prospective Cohort Study on Cumulative Fluid Balance and Outcome in Critically Ill Children Using a Restrictive Fluid Protocol

Summary: This prospective cohort study examined the association between cumulative fluid balance and clinical outcomes in a pediatric intensive care unit practicing a restrictive fluid protocol. The study found that there was no significant difference in PICU mortality and morbidity among patients with cumulative fluid balance within 3%. However, patients with positive fluid balance had a longer stay in the intensive care unit and hospital.

Factors associated with mortality of pediatric sepsis patients at the pediatric intensive care unit in a low-resource setting

Advances in Neonatal Acute Kidney Injury

Preventive strategies and new technologies have shown promise in the management of AKI, and there is a growing recognition of the importance of kidney function in the development and injury of other organs. Future research is needed to focus on optimal follow-up strategies for neonates with a history of AKI to detect chronic kidney disease.

Restrictive fluid management versus usual care in acute kidney injury (REVERSE-AKI): a pilot randomized controlled feasibility trial

Continuous Renal-Replacement Therapy in Critically Ill Children: Practice Changes and Association With Outcome

Summary: This study evaluated practice changes and outcomes of critically ill children receiving continuous renal-replacement therapy in a large single-center PICU over a 10-year period. The study found improvements in patient survival rate by implementing early initiation of therapy, reducing fluid overload, and using regional citrate anticoagulation. Ongoing evaluation and quality improvement in the practice of continuous renal-replacement therapy for pediatric patients is still needed.

Timing and Clinical Significance of Fluid Overload in Pediatric Acute Respiratory Distress Syndrome*

Summary: This retrospective cohort study analyzed data of 723 intubated children with acute respiratory distress syndrome between 2011 and 2019 in a single tertiary care PICU. The study found that fluid overload after day 4 of acute respiratory distress syndrome onset was associated with worse outcomes. Elevated angiopoietin-2 levels predicted subsequent fluid overload.

Post-operative fluid overload as a predictor of hospital and long-term outcomes in a pediatric heart transplant population

Summary: The study found that fluid overload is common after pediatric heart transplant and is associated with worse clinical outcomes. Shorter donor ischemic time and longer cardiopulmonary bypass time may increase the risk of fluid overload. Fluid overload greater than 20% is a statistically significant independent predictor of mortality, ventilation time, and PICU length of stay.
