Diabetic retinopathy as the leading cause of blindness and early predictor of cascading complications-risks and mitigation

Summary: Proliferative diabetic retinopathy (PDR), a complication of diabetes mellitus (DM), is a major cause of blindness. The current screening process for DR is not effective in detecting the disease early enough. PDR is characterized by ocular damage, inflammation, neovascularization, and reduced visual field. It can also predict other severe diabetic complications. This article focuses on the use of multi-omic tear fluid analysis for DR prognosis and PDR prediction, and suggests personalized medicine approaches for prevention and management of DR.

Arterial Stiffness, Genetic Risk, and Type 2 Diabetes: A Prospective Cohort Study

Arterial stiffness precedes hypertension and metabolic risks in youth: a review

Summary: Arterial stiffness is a strong predictor of cardiovascular events and all-cause mortality in middle-aged and old adults. However, it has been limited to being an intermediate marker of atherosclerotic cardiovascular events in adolescents and young adult studies. The lack of normative longitudinal data and repeated gold-standard assessments in the young population has led to a significant knowledge gap in its clinical utility. This review summarizes recent evidence in a large adolescent population, supporting the value of arterial stiffness as a novel risk factor for hypertension, overweight/obesity, and insulin resistance, and provides suggestions for incorporating arterial stiffness measures into routine pediatric and young adult clinical practice.

Studies have shown that structural stiffness predicts outcomes, with elastic arteries being most sensitive to blood pressure and age. Stiffness is considered an index of vascular aging and affects brain, kidneys, and heart function. Reduction of stiffness through antihypertensive treatments and preventive drugs may have additional benefits beyond lowering blood pressure. Newer anti-inflammatory drugs also have effects on stiffness independent of blood pressure.
