Second endoscopic retrograde cholangiopancreatography after failure of initial biliary cannulation: A single institution retrospective experience

Summary: This study retrospectively analyzed the outcomes of repeating ERCP in patients with failed initial biliary cannulation. It found that a second ERCP appears to be safe and effective, with a higher success rate and lower mortality compared to patients without a second ERCP. Preoperative normal serum bilirubin levels may be a predictive factor for cannulation failure in a second ERCP.

Summary: Recommendations for biliary cannulation include measures to prevent difficult papilla situations and techniques to address difficult biliary cannulation. Rescue techniques such as needle-knife precut and double-guidewire technique are important in managing DBC cases where conventional methods fail. EUS-guided biliary drainage is increasingly used as an alternative to percutaneous transhepatic biliary drainage in certain situations.

Influence of a novel classification of the papilla of Vater on the outcome of needle-knife fistulotomy for biliary cannulation

Summary: The study introduced a novel endoscopic classification system for Papilla of Vater, showing high reproducibility among experienced ERCPists but requiring further training for nonexperts. Using the new system, different types of papillae were identified to be significantly associated with reduced efficacy of NKF and prolonged time for successful biliary cannulation.
