AGREE II appraisals of clinical practice guidelines in rehabilitation showed poor reporting and moderate variability in quality ratings when users apply different cuff-offs: a methodological study

Summary: This study analyzed the reporting characteristics of AGREE II appraisals in rehabilitation and found that applying different cut-offs for quality ratings of CPGs may lead to significant variability in ratings. The application of the most frequent cut-offs resulted in a notable proportion of CPGs shifting their ratings from low to high quality, highlighting the importance of selecting the highest quality CPGs in rehabilitation practice.

Guidelines rarely used GRADE and applied methods inconsistently: A methodological study of Australian guidelines

Summary: The study found that out of 240 guidelines authored by 51 different organizations, 15 followed the GRADE methodology. The application of GRADE methods varied between guidelines, with some misreporting and altering aspects of the process. Guidelines that closely adhered to the guidance from the GRADE Working Group scored higher in rigor of development scores. The use of GRADE in Australian guidelines is increasing, but strategies to increase uptake and reporting within the guideline community need to be explored.

Summary: A systematic review of clinical practice guidelines (CPGs) developed in China and published in medical journals between 2014 and 2018 found improvements in both methodological and reporting quality over time, although they are still below the international average. Subgroup analyses showed that CPGs using evidence grading systems or reporting receiving funding tended to have higher quality in both aspects.

The injustice of unfit clinical practice guidelines in low-resource realities

Summary: In order to end the international crisis of preventable deaths in low-income and middle-income countries, evidence-informed and cost-efficient health care along with contextually relevant clinical practice guidelines are urgently needed. There are significant gaps between international recommendations and realistic best practices, highlighting the importance of developing more realistic global guidelines. Developing and adapting high-quality clinical practice guidelines at national and subnational levels in low-income and middle-income countries, through co-creation with end-users, is essential for ensuring successful implementation.
