Early kinetics of donor-derived cell-free DNA after transplantation predicts renal graft recovery and long-term function

Summary: This study proposes the use of donor-derived cell-free DNA (ddcfDNA) as an objective tool to assess the severity of ischemia-reperfusion injury (IRI) during kidney transplantation. The results show that ddcfDNA at 24 hours after transplantation is associated with delayed graft function (DGF) and 6-month estimated glomerular filtration rate. Additionally, ddcfDNA at Day 7 after transplantation is associated with dialysis duration in DGF patients and worse 7-year graft survival probability. Early normalization of ddcfDNA is also linked to improved graft survival probability.

Donor-derived cell free DNA as a biomarker in kidney transplantation

Donor-derived cell-free DNA (dd-cfDNA) has emerged as a new biomarker for this purpose. This review examines the role of dd-cfDNA compared to creatinine and donor-specific antibodies in early rejection detection, as well as its potential use in other organ transplants and future directions as a diagnostic tool.
