Therapeutic Drug Monitoring of Ganciclovir in Cytomegalovirus-Infected Patients With Solid Organ Transplants and Its Correlation to Efficacy and Toxicity

Summary: This study compared the efficacy, toxicity, and occurrence of resistance between target trough levels of 1-2 mg/L and 2-4 mg/L for cytomegalovirus infection. The results showed no significant differences in efficacy and toxicity between the two groups, but a correlation was found between drug levels and lymphopenia.

Summary: Kidney transplant recipients are at increased risk for CMV infection due to necessary immunosuppression, with prophylaxis and preemptive therapy being common management approaches. Despite prevention measures, a significant percentage of patients still develop CMV infection, with age and D+/R- serostatus being consistent risk factors. CMV infection is associated with increased mortality and graft loss. New strategies may be necessary to optimize CMV management.

Clinical Features, Endoscopic Findings, and Predictive Factors for Mortality in Tissue-Invasive Gastrointestinal Cytomegalovirus Disease between Immunocompetent and Immunocompromised Patients

Summary: This study compared the clinical manifestations, endoscopic features, treatment outcomes, and predictors for inhospital mortality of tissue-invasive gastrointestinal cytomegalovirus (TI-GI CMV) between immunocompromised and immunocompetent patients. Patients with HIV infection, malignancy, or receiving immunosuppressive agents were considered immunocompromised. The study found that ICU admission, sepsis/shock, malnutrition, and receiving chemotherapy were independent factors for inhospital mortality, while antiviral treatment for more than 14 days improved survival.
