Summary: This article describes the clinical characteristics of a rare mutation (c.-5+5 G > A) in the SERPINA1 gene, which results in alpha1-antitrypsin (AAT) deficiency. The patient, a 77-year-old female with severe chronic obstructive pulmonary disease (COPD) and panlobular emphysema, experienced respiratory failure on exertion. The AAT level in her serum was significantly low (0.1 g/L), indicating null mutation. Prompt screening and identification of AAT deficiency would enable early intervention.

Registry-based cohort study of alpha-1 antitrypsin deficiency prevalence, incidence and mortality in Denmark 2000-2018

Summary: Based on data from the Danish National Patient Registry and the Danish Civil Registration System, the prevalence of diagnosed alpha-1 antitrypsin deficiency (dAATD) in Denmark was found to be 12.9 per 100,000 persons, with an incidence rate of 0.90 per 100,000 person-years. Mortality was significantly higher for patients with dAATD compared to the general population, especially for children.

Frequency of alpha-1 antitrypsin deficiency and unexpected results in COPD patients in Turkey; rare variants are common

Summary: This study investigated the frequency of genotypic alpha 1-antitrypsin (alpha 1-AT) variants in COPD patients in our country. It found that the percentage of homozygous Pi*ZZ patients was 0.51%, but when including heterozygous alpha 1-AT gene variants and single point mutations, the frequency was 7.1%. Interestingly, the Pi*S variant was not detected, but rare variants were found more frequently than expected.

Feasibility of a genotyping system for the diagnosis of alpha1 antitrypsin deficiency: a multinational cross-sectional analysis

Summary: This study reported the performance of a multinational genotyping test for alpha1 antitrypsin deficiency (AATD) using dried blood spots and buccal swabs sent by post or courier. The results showed that this diagnostic system can successfully conduct genotyping for AATD simultaneously in different countries, improving the timely diagnosis of the condition.
