Impact of thoracic duct resection during radical esophagectomy on oncological and survival outcomes: Systematic review

Prognosis of Patients with Esophageal Carcinoma After Routine Thoracic Duct Resection A Propensity-matched Analysis of 12,237 Patients Based on the Comprehensive Registry of Esophageal Cancer in Japan

Summary: This study aimed to investigate whether routine thoracic duct resection improves the prognosis of patients with esophageal cancer after radical esophagectomy. The study found that thoracic duct resection did not improve the overall survival of patients with esophageal cancer and may even lead to distant metastases. Therefore, prophylactic thoracic duct resection is not recommended in patients with esophageal cancer.

Summary: This study aimed to examine whether indiscriminate thoracic duct resection improved the prognosis of patients with esophageal squamous cell carcinoma (ESCC). The results showed that thoracic duct resection did not contribute to improved survival and was associated with more organ metastases compared to thoracic duct preservation. Therefore, indiscriminate thoracic duct resection should be avoided in patients with ESCC.

Diagnosis and treatment for gastro-oesophageal cancer in England and Wales: analysis of the National Oesophago-Gastric Cancer Audit (NOGCA) database 2012-2020

Summary: National population data for patients with oesophagogastric cancer in England and Wales between 2012 and 2020 were analyzed. Despite failures in early diagnosis and increasingly comorbid patients, improvements in surgical and oncological outcomes were noted. Regression analysis showed that more recent care and higher unit volume were significantly associated with better surgical outcomes.

The Presence of Metastatic Thoracic Duct Lymph Nodes in Western Esophageal Cancer Patients: A Multinational Observational Study

Summary: This study assessed the presence and metastatic involvement of thoracic duct lymph nodes (TDLNs) in a Western population with esophageal adenocarcinoma. The results demonstrated the presence of metastatic TDLNs in adenocarcinomas, providing evidence to routinely extend the thoracic lymphadenectomy with resection of the thoracic duct compartment during esophagectomy for esophageal cancer.

Robot-assisted Versus Conventional Minimally Invasive Esophagectomy for Resectable Esophageal Squamous Cell Carcinoma Early Results of a Multicenter Randomized Controlled Trial: the RAMIE Trial

Summary: This study compared the outcomes of robot-assisted minimally invasive esophagectomy (RAMIE) and conventional minimally invasive esophagectomy (MIE) in the treatment of esophageal squamous cell carcinoma (ESCC). The results showed that RAMIE had shorter operation time and better lymph node dissection in patients who received neoadjuvant therapy. RAMIE and MIE had similar rates of complications and postoperative complications.

Prophylactic thoracic duct obliteration and resection during esophagectomy: What is the impact on perioperative risks and long-term survival? A systematic review and meta-analysis

Summary: There is no agreement on the benefits of prophylactic thoracic duct ligation (TDL), with or without resection, during esophagectomy for cancer patients. The effects of these procedures on postoperative complications and overall survival remain unclear. This systematic review of 16 articles found that TDL did not affect short- and long-term outcomes. However, thoracic duct resection increased postoperative chylothorax and overall complications, without improving survival.
