Clinical course and management of adverse events after endoscopic resection of superficial duodenal epithelial tumors: Multicenter retrospective study

Summary: This study aimed to clarify the clinical course and management of adverse events (AEs) after endoscopic resection (ER) for superficial duodenal epithelial tumors (SDETs). The results showed that delayed perforation had a higher surgical conversion rate compared to other AEs, and involvement of the major papilla and delayed perforation were significant risk factors for surgical conversion after AEs.

Recent approach for preventing complications in upper gastrointestinal endoscopic submucosal dissection

Summary: This article introduces the methods and techniques for preventing complications during upper gastrointestinal endoscopic submucosal dissection (ESD). In esophageal and gastric ESD, intraoperative perforation is a major complication that can be prevented by avoiding blind dissection and using traction-assisted ESD. Delayed bleeding is the most common complication in gastric ESD, and a prediction model (BEST-J score) has been established to assess the risk of delayed bleeding. Vonoprazan >= 20 mg/day is currently the only reliable method for reducing delayed bleeding in gastric ESD. Duodenal ESD has a higher frequency of complications, such as perforation and delayed bleeding, but advancements in devices and techniques have decreased the occurrence. Several ESD techniques, including the use of the distal tips of the Clutch Cutter, can help prevent intraoperative perforation. Standardization of duodenal ESD and further studies are still needed.

Efficacy of endoscopic preventive procedures to reduce delayed adverse events after endoscopic resection of superficial nonampullary duodenal epithelial tumors: a meta-analysis of observational comparative trials
