Does HLA matching matter in the modern era of renal transplantation?

Summary: Children with end-stage kidney disease are best offered a well-matched pre-emptive kidney transplant, with international allocation schemes often prioritizing pediatric patients and younger donors. HLA matching can be challenging and may lead to longer waiting times, but living donation still provides the best outcomes for allograft survival, even in cases of highly mismatched living donors.
