Management and Outcomes of Pathologic Upstaging of Clinical Stage I Rectal Cancers: An Exploratory Analysis

Summary: In this study, it was found that approximately one in four patients with stage I rectal cancer had T3 or nodal disease detected on operative pathology. Despite receiving adjuvant therapy, patients with occult nodal disease had worse outcomes, with systemic recurrence being more common than local recurrence.

Predictors and Outcomes of Upstaging in Rectal Cancer Patients Who Did Not Receive Preoperative Therapy

Summary: The study examined patients with clinical stage I rectal adenocarcinoma who did not receive preoperative chemoradiation. It found that tumor upstaging occurred in 10.6% of patients and nodal upstaging occurred in 20.8% of patients. Upstaged patients had higher grade tumors and positive margins. Survival was worse in upstaged patients, but improved with postoperative chemotherapy and/or chemoradiation.

Outcomes for endoscopic submucosal dissection of pathologically staged T1b esophageal cancer: a multicenter study

Summary: This study evaluated the technical outcomes and recurrence rates of ESD in treating T1b esophageal cancer. It found that EUS staging had poor accuracy, R0 resection rates were low, and R1 resection was associated with recurrent disease. Close surveillance is necessary for noncurative ESD resection patients who cannot undergo surgery.

Tumor size predicts worse prognosis in esophagogastric junction adenocarcinoma

Summary: This retrospective study evaluated 188 patients with resected locally advanced adenocarcinoma of the esophagogastric junction (AEGJ) and found that tumor size and location were associated with prognosis. The study found that tumor size < 5 cm and location in the G subgroup were associated with better disease-free survival, while high histological grade and advanced pT status were associated with worse overall survival.

The Discordance of Clinical and Pathologic Staging in Locally Advanced Gastric Adenocarcinoma

Summary: This study revealed significant discordance between clinical and pathologic staging in resectable locally advanced gastric adenocarcinoma. Predictors of up-staging included younger age, male sex, poor tumor grade, positive margin status, and days from diagnosis to surgery. Patients who were up-staged had a lower survival rate compared to those who were accurately staged.
