ISPD peritonitis guideline recommendations: 2022 update on prevention and treatment

Summary: Peritoneal dialysis (PD)-associated peritonitis is a serious complication of PD, and its prevention and treatment are crucial for reducing patient morbidity and mortality. The ISPD 2022 updated recommendations provide revised definitions, new peritonitis categories and outcomes, and updated targets for overall peritonitis rate and percentage of patients free of peritonitis. The recommendations also cover management of contamination, antibiotic prophylaxis, PD training, and modifiable peritonitis risk factors. Additionally, there are specific recommendations for antibiotic selection, treatment of peritonitis due to specific microorganisms, and adjunctive therapy.

Summary: This study reviewed 62 episodes of peritonitis caused by NF-GNB, finding a high proportion of biofilm producer lineages and a significant association between preexisting exit-site infection and nonresolution. Despite higher in vitro susceptibility of Pseudomonas compared to other NF-GNB, there was a similar low resolution rate among the episodes, suggesting that bacterial virulence factors such as biofilms may worsen the outcome.

Peritoneal dialysis-related peritonitis caused by gram-negative organisms: ten-years experience in a single center

Summary: Over the past 10 years, there has been an increase in the proportion of gram-negative bacterial peritonitis, with E. coli being the most common pathogen. Characteristics of gram-negative bacterial peritonitis include higher effluent WCC on the first day, longer time required for WCC to normalize, and higher level of CRP. Prognosis of gram-negative bacterial peritonitis is worse.
