The uncertain role of substandard and falsified medicines in the emergence and spread of antimicrobial resistance

Pharmacopeial quality of artemether-lumefantrine anti-malarial agents in Uganda

Summary: This study assessed the pharmacopeial quality of artemether-lumefantrine (AL) in low and high malaria transmission settings in Uganda. The study found that 18.9% of the AL samples were of substandard quality, with a higher prevalence in high malaria transmission settings. Continuous surveillance and monitoring of the quality of artemisinin-based anti-malarials is necessary.

Advances and roadblocks in the treatment of malaria

Summary: The use of artesunate and artemisinin combination therapies (ACTs) in the treatment of severe and uncomplicated malaria respectively has been a significant advancement in antimalarial therapeutics. However, resistance to these treatments has emerged in Southeast Asia. Other treatments such as primaquine and tafenoquine have shown promise in preventing relapse, and targeted malaria elimination has been effective in certain areas. However, global progress towards malaria elimination has been slow, and it is crucial to prioritize the preservation of current antimalarials as new drugs are not expected to become widely available in the near future.

Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Antibiotic combinations reduce Staphylococcus aureus clearance

Summary: The study finds that combinations of antibiotics have different effects on long-term bacterial clearance, often showing reciprocal suppression. The efficacy of clearance decreases as more drugs are added, except for specific drugs targeting non-growing persisters. Competition experiments show that reciprocal suppressive drug combinations can select against resistance, even against methicillin-resistant Staphylococcus aureus. Therefore, systematically studying the long-term clearance efficacy of drug combinations is crucial for designing more effective and resistance-proof multidrug regimens.

Frequency of bystander exposure to antibiotics for enteropathogenic bacteria among young children in low-resource settings

Summary: Children in low-resource settings often carry enteric pathogens without symptoms and are frequently exposed to antibiotics unknowingly. This study found that most antibiotic exposures were not targeted towards the pathogens causing the infection. Respiratory infections and diarrheal illnesses were the main causes of pathogen exposure. Bystander exposure of E. coli to antibiotics was associated with community-level resistance. Antimicrobial stewardship and illness-prevention interventions in low-resource settings could significantly reduce bystander selection and contribute to antimicrobial resistance prevention.

The quality of veterinary medicines and their implications for One Health

Summary: Over the past decade, declining efficacy of artemisinin combination therapies (ACTs) within Southeast Asia has been observed, which are recommended by the World Health Organization (WHO) as the last line of defense against malaria. The acceleration of antimicrobial resistance (AMR) due to substandard drug treatments has been documented, with nearly 10% of medical products failing quality standards in low- and middle-income countries according to WHO in 2017. A tissue culture-based approach has been developed to test possible connections between substandard treatment and the spread of ACT-resistant blood stage forms of P. falciparum. Through sequencing of pfk13, a molecular marker predictive for ART resistance, conditions favoring emergence of ARTR parasites are being defined.
