Cerebral Hemorrhage: Pathophysiology, Treatment, and Future Directions

Summary: This review article provides an overview of the epidemiology, cause, mechanisms of injury, current treatment strategies, and future research directions of intracerebral hemorrhage (ICH). The incidence of hemorrhagic stroke has increased globally in the past 40 years, with changes in the cause over time due to improved hypertension management and increased use of anticoagulants. Preclinical and clinical trials have shed light on the underlying cause and mechanisms of injury from ICH, including the complex interaction between edema, inflammation, iron-induced injury, and oxidative stress. Although several trials have investigated the optimal medical and surgical management of ICH, there has been no clear improvement in survival and functional outcomes. Ongoing research into novel approaches for ICH management offers hope for reducing the devastating impact of this disease in the future.

Inpatient mortality and healthcare resource utilization of nontraumatic intracerebral hemorrhage complications in the US

Summary: This study quantified the incidence and healthcare resource utilization for major complications in nontraumatic, primary intracerebral hemorrhage patients. The results showed that more than 42% of patients with ICH developed complications during initial admission, resulting in high inpatient mortality rates and considerable healthcare costs. Hydrocephalus was identified as one of the complications with the highest inpatient mortality rates and healthcare costs.

Prolonged Mechanical Ventilation in Patients with Deep-Seated Intracerebral Hemorrhage: Risk Factors and Clinical Implications

Summary: This study aimed to determine the impact of prolonged mechanical ventilation (PMV) on mortality in patients with deep-seated intracerebral hemorrhage (ICH) and identify associated risk factors. The results showed that ICH volume >30 mL and admission SOFA score > 5 were significant and independent predictors for the need of PMV in patients with deep-seated ICH.

Disability and the risk of subsequent mortality in elderly: a 12-year longitudinal population-based study

Summary: Assessing the impact of disability on mortality among the elderly is important for healthy aging. A longitudinal study using data from the Chinese Longitudinal Healthy Longevity Study found that disability increases the risk of all-cause death in the elderly, particularly those with chronic diseases and in the younger old-age group. Further research is needed to understand how to effectively prevent disability in the older population.

Long-term survival after primary intracerebral hemorrhage: A population-based case-control study spanning a quarter of a century

Summary: The study revealed a high mortality rate of 81.1% among ICH patients, with cerebrovascular disease being the main cause of death. Long-term survivors had smaller ICH volume, and even 10 years after the index event, ICH patients still had a higher mortality rate compared to controls.
