Dosimetric study between a single isocenter dynamic conformal arc therapy technique and Gamma Knife radiosurgery for multiple brain metastases treatment: impact of target volume geometrical characteristics

Summary: The study compared linac-based mono-isocentric radiosurgery and Brainlab Elements Multiple Brain Mets (MBM) SRS with the Gamma Knife for treating brain metastases, analyzing the dosimetric impact of target volume geometric characteristics. The results showed that for smaller lesions, MBM had slightly improved selectivity and spared healthy tissues, while linac-based radiosurgery had better dose fall-off for larger volumes.

Summary: This evidence report synthesized available evidence on radiation therapy for brain metastases, finding no significant difference in overall survival or death due to brain metastases when combining SRS plus WBRT compared to SRS alone or WBRT alone. Radiation therapy after surgery did not improve overall survival compared to surgery alone. More data on patient-relevant outcomes such as quality of life, functional status, and cognitive effects are needed.

Summary: This study establishes the equivalence between flattened and unflattened beam models and different delivery techniques for stereotactic radiosurgery in small tumor volumes. Dose conformity, heterogeneity, dose fall-off characteristics, and organ at risk doses show no or very little variation. Unflattened beams could offer only limited time advantage due to excess dose rate compared to flattened beams.

Single isocenter treatment planning techniques for stereotactic radiosurgery of multiple cranial metastases

Summary: This study compared two single isocenter radiosurgery planning techniques, VMAT and DCA, in terms of dosimetric and delivery performance. Results showed that DCA plans were dosimetrically superior to VMAT plans, with reduced V-12Gy and associated risk for S-NEC. Maximum doses to important OARs also showed significant improvement with DCA plans.
