The pharmacokinetics and safety of lidocaine in liver cancer patients undergoing hepatic resection

Chaoqun He et al.

Summary: The purpose of this study was to investigate the pharmacokinetics and safety of continuous lidocaine infusion during hepatectomy in liver cancer patients. Thirty-five patients undergoing laparoscopic hepatectomy were included in the study. Lidocaine concentrations were within the safe range, except for one patient. The mean half-life, time to maximum observed concentration, and maximum observed concentration of lidocaine were 3.96 h, 2.85 h, and 2030 ng/mL, respectively.

Postoperative delirium, neuroinflammation, and influencing factors of postoperative delirium: A review

M. Z. Xiao et al.

Summary: Postoperative delirium (POD) is an acute cognitive dysfunction characterized by memory impairment and disturbances in consciousness, which can prolong hospital stay and increase 1-month mortality rate. Neuroinflammation is an important pathogenic mechanism mediating microglial activation and synaptic remodeling in POD. Understanding and preventing risk factors associated with neuroinflammation may help in preventing POD.

Comparison of the Effects of Dexmedetomidine and Lidocaine on Stress Response and Postoperative Delirium of Older Patients Undergoing Thoracoscopic Surgery: A Randomized Controlled Trial

Yuan Lai et al.

Summary: This study investigated the effects of intraoperative intravenous lidocaine or dexmedetomidine infusion on inflammatory factors and cognitive function in patients undergoing thoracoscopic surgery. The results showed that intraoperative continuous intravenous lidocaine or dexmedetomidine infusion reduced surgical stress and inflammatory responses, but had no significant effect on postoperative cognitive function.

Comparison of the hemodynamic effects of opioid-based versus lidocaine-based induction of anesthesia with propofol in older adults: a randomized controlled trial

Sarah M. Amin et al.

Summary: This study aimed to compare the hemodynamic profile of lidocaine and fentanyl during propofol induction of general anesthesia. The results showed that the lidocaine group had a lower incidence of hypotension during surgery compared to the fentanyl group, and did not require the use of norepinephrine for treatment. In conclusion, lidocaine-based regimen for induction of anesthesia reduces the risk of postinduction hypotension in older patients compared to the fentanyl-based regimen.

Evidence-based strategies to reduce the incidence of postoperative delirium: a narrative review

C. J. Swarbrick et al.

Summary: Delirium is a common complication in older adults after surgery due to vulnerability of cerebral functioning. Early identification of high-risk patients provides an opportunity for intervention and shared decision-making. Multi-component interventions involving risk factor modification and geriatrician input are effective in preventing delirium.

Postoperative delirium and changes in the bloodebrain barrier, neuroinflammation, and cerebrospinal fluid lactate: a prospective cohort study

Jennifer Taylor et al.

Summary: This study found that changes in cerebrospinal fluid/plasma albumin ratio and plasma S100B during the perioperative period were associated with the severity of delirium. The breakdown of the blood-brain barrier was identified as a cause of postoperative delirium. Blood loss was also found to be related to the occurrence and severity of delirium.

Effect of Regional vs General Anesthesia on Incidence of Postoperative Delirium in Older Patients Undergoing Hip Fracture Surgery The RAGA Randomized Trial

Ting Li et al.

Summary: A randomized clinical trial in Southeastern China involving 950 patients aged 65 years and older undergoing hip fracture surgery found that regional anesthesia without sedation did not significantly reduce the incidence of postoperative delirium compared with general anesthesia. The study also investigated secondary outcomes such as delirium severity, duration, and subtype, as well as postoperative pain score, length of hospitalization, mortality, and complications.

Intraoperative lidocaine infusion in patients undergoing pancreatectomy for pancreatic cancer: a mechanistic, multicentre randomised clinical trial

Hao Zhang et al.

Summary: Intraoperative infusion of lidocaine did not improve overall or disease-free survival in patients undergoing pancreatectomy for pancreatic cancer. However, it reduced the use of intraoperative opioids and circulating NETs formation after surgery, but had no effect on tumor-associated NETs.

Postoperative delirium in geriatric patients with hip fractures

Yang Chen et al.

Summary: Postoperative delirium (POD) is a common complication in geriatric patients with hip fractures, associated with poorer functional recovery, longer hospital stays, and higher mortality rates. Various factors such as advanced age, preoperative cognitive impairment, comorbidities, perioperative polypharmacy, and delayed surgery increase the risk of developing POD. Effective interventions such as perioperative risk prediction, avoidance of certain medications, and orthogeriatric comprehensive care can help prevent POD. Anesthesia technique choice does not seem to significantly impact the incidence of postoperative delirium in geriatric patients with hip fractures. There are limited pharmaceutical measures available for POD treatment, with dexmedetomidine and multimodal analgesia showing potential effectiveness but also considering adverse complications when using antipsychotics. In conclusion, perioperative risk intervention based on orthogeriatric comprehensive care is the most effective strategy for preventing postoperative delirium in geriatric patients with hip fractures.

Impact of lidocaine on hemodynamic and respiratory parameters during laparoscopic appendectomy in children

Maciej Kaszynski et al.

Summary: This study assessed the effects of systemic lidocaine administration on ventilatory and circulatory parameters, as well as the impact of pneumoperitoneum on the cardiopulmonary system during laparoscopic appendectomy in children. The results showed that lidocaine administration did not alleviate circulatory and respiratory alterations during the surgery.

The use of intravenous lidocaine for postoperative pain and recovery: international consensus statement on efficacy and safety

I. Foo et al.

Summary: The use of intravenous lidocaine in peri-operative pain management should be considered carefully due to its potential benefits and risks, especially for patients with comorbidities. Proper approval from hospital medication governance systems and informed consent from patients are crucial before utilizing this 'high-risk' medicine. Dosage recommendations and timing of administration in relation to other local anesthetic interventions should also be strictly followed.
