Elevated glutamate and decreased glutamine levels in the cerebrospinal fluid of patients with MELAS syndrome

Maria Paz Guerrero-Molina et al.

Summary: This study found that levels of glutamate were significantly elevated and glutamine levels were significantly decreased in the cerebrospinal fluid of patients with MELAS syndrome. Additionally, levels of alanine, branched-chain amino acids, and lactate were found to be significantly higher in MELAS patients. These findings suggest an altered glutamate-glutamine cycle and upregulated lactate-alanine and branched-chain amino acid-glutamate cycles, which may have therapeutic implications in MELAS syndrome.

In Vivo 13C Magnetic Resonance Spectroscopy for Assessing Brain Biochemistry in Health and Disease

Pravat K. Mandal et al.

Summary: Magnetic resonance spectroscopy (MRS) is a non-invasive technique that helps to understand the biochemistry of the brain. C-13 MRS can detect specific neurochemicals and their correlation with neuronal function, providing insights into neurodegenerative diseases. It can also investigate changes in brain metabolism and reflect the real-time production and alterations of key brain metabolites in psychiatric and neurological disorders.

MRI Features of Stroke-Like Episodes in Mitochondrial Encephalomyopathy With Lactic Acidosis and Stroke-Like Episodes

Weiqin Cheng et al.

Summary: MELAS, a cause of stroke-mimicking diseases, affects individuals before the age of 40. Gene mutations in mitochondrial DNA or nuclear DNA lead to a wide range of clinical symptoms and imaging findings. MRI is crucial for detecting stroke-like lesions and monitoring disease progression and treatment efficacy.

Brain MRS correlates with mitochondrial dysfunction biomarkers in MELAS-associated mtDNA mutations

Laura L. Gramegna et al.

Summary: This study investigated correlations between brain proton magnetic resonance spectroscopy findings with serum biomarkers and mitochondrial DNA mutations in patients with MELAS and MSS. The results showed relationships between peripheral and biochemical markers of mitochondrial dysfunction and brain neurodegeneration markers, supporting their use as signatures of these diseases.

Clinical features, pathogenesis, and management of stroke-like episodes due to MELAS

Syuichi Tetsuka et al.

Summary: MELAS should be considered in the differential diagnosis of acute ischemic stroke due to similar onset patterns and neurological symptoms. Advances in neuroimaging have facilitated differentiation between stroke and MELAS. Potential preventive treatments for stroke-like episodes in MELAS include supplementation with L-arginine or taurine, while new gene therapies offer promise for future treatment options.

Metabolic abnormality in acute stroke-like lesion and its relationship with focal cerebral blood flow in patients with MELAS: Evidence from proton MR spectroscopy and arterial spin labeling

Rong Wang et al.

Summary: This study utilized 1HMRS and ASL to investigate metabolic alterations and cerebral blood flow in MELAS patients with acute SLLs. The results showed that MELAS patients exhibited metabolic abnormalities and increased rCBF in acute SLEs compared to normal controls, with specific differences in metabolite levels. Additionally, a positive correlation was found between lactate concentration and focal rCBF in acute SLLs of MELAS patients.
