Different FT3/TSH correlation in acquired and congenital hypothyroid patients reveals a different hypothalamic set-point

Summary: This study aimed to investigate the differences in thyroid hormone replacement therapy between acquired and congenital hypothyroid patients. It found that adult congenital hypothyroid patients require a higher dose of levo-thyroxine than thyroidectomized patients. Additionally, the correlation between TSH and FT3 differed between the two groups, suggesting a difference in hypothalamic-pituitary-thyroid axis regulation.

Congenital hypothyroidism can be caused by defects in transcription factors or thyroid stimulating hormone receptor function, while dyshormonogenic hypothyroidism is usually due to mutations in genes related to thyroidal iodide transport, organification, or iodotyrosine synthesis and recycling. Disorders of thyroid hormone signaling can be caused by defects in membrane transporters, deficiency of deiodinases, or pathogenic variants in TR alpha or TR beta.
