Clinical features, etiologies, and outcomes in adult patients with meningoencephalitis requiring intensive care (EURECA): an international prospective multicenter cohort study

Summary: This study aimed to characterize the outcomes of severe meningoencephalitis patients requiring intensive care. A multicenter international cohort study was conducted from 2017 to 2020 in 68 centers across 7 countries. Among the enrolled patients, 50.5% had a poor functional outcome at 3 months, and the independent factors associated with this outcome included age > 60 years, immunodepression, delayed ICU admission, low Glasgow Coma Scale score, motor deficits, respiratory and cardiovascular complications, while administration of certain medications on ICU admission was protective.

Pneumococcal Meningitis in Adults: A Prospective Nationwide Cohort Study Over a 20-year Period

Summary: The epidemiology and treatment of pneumococcal meningitis in the Netherlands has changed with the implementation of conjugate vaccines and adjunctive dexamethasone therapy, leading to a decrease in incidence of certain serotypes and an increase in the use of dexamethasone therapy. Although dexamethasone therapy was associated with a favorable outcome, pneumococcal meningitis still remains associated with a high rate of mortality and morbidity, highlighting the need for continuous efforts in identifying new adjunctive treatments.

Summary: The presence of bacteraemia in severe pneumococcal community-acquired pneumonia did not have an impact on the mortality of immunocompetent Caucasian adults admitted to intensive care. Factors associated with mortality were not significantly different between patients with and without bacteraemia, and there were no specific prognostic factors identified for bacteraemic patients.

Burden of pneumococcal pneumonia requiring ICU admission in France: 1-year prognosis, resources use, and costs

Summary: Community-acquired pneumonia, especially pneumococcal CAP, is associated with high rates of ICU admission, mortality, and costs. Factors such as age, malignancies, liver diseases, and severity of illness upon ICU admission are associated with increased mortality risk. Older age, malignant and chronic liver diseases are associated with increased mortality, while chronic cardiac failure and respiratory disease are associated with increased costs.
