Facial nerve paresis in the course of masked mastoiditis as a revelator of GPA

Summary: This study presented 6 cases of facial nerve palsy and masked mastoiditis as revelators of localized GPA, evaluating the utility of ACR/EULAR 2017 criteria. Patients exhibited unilateral facial nerve palsy and hearing loss, with some showing rapid progression of the disease. The importance of early diagnosis and treatment for a better prognosis was highlighted.

Summary: Granulomatosis with polyangiitis is a highly variable disease that commonly involves the upper airways but may also present solely with symptoms related to the pituitary gland. Clinicians should have a low threshold to investigate for granulomatosis with polyangiitis in patients with therapy-resistant otorrhea. Patients may present with a complex set of symptoms, and involving different specialists can lead to a faster diagnosis.
