Adverse effects of antibiotics in children with cancer: are short-course antibiotics for febrile neutropenia part of the solution?

Coen Butters et al.

Summary: Febrile neutropenia is a common complication in children with cancer or undergoing hematopoietic stem cell transplantation. Repeated episodes of febrile neutropenia lead to cumulative exposure to broad-spectrum antibiotics, which can have serious adverse effects. Short-course antibiotics may be a solution, even in high-risk cases.

Summary: This article aims to update a clinical practice guideline for the management of fever and neutropenia in pediatric patients with cancer and hematopoietic cell transplantation recipients. The guideline panel reviewed new randomized controlled trials and classified the evidence quality. Changes were made based on the latest evidence.

The incidence and impact of clostridioides difficile infection on transplant outcomes in acute leukemia and MDS after allogeneic hematopoietic cell transplant-a CIBMTR study

Muthalagu Ramanathan et al.

Summary: This study examined the data of patients who underwent the first allogeneic hematopoietic cell transplantation (alloHCT) for AML, ALL, or MDS between 2013 and 2018 in the US. The analysis showed that CDI was associated with inferior overall survival and an increased risk of infection-related mortality.

Early stool microbiome and metabolome signatures in pediatric patients undergoing allogeneic hematopoietic cell transplantation

Caitlin W. Elgarten et al.

Summary: This study analyzed the pretransplant stool microbiome of pediatric allogeneic HCT patients at four centers. The findings showed significant dysbiosis in patients early in the transplant course, with differences compared to healthy controls. Strong correlations between stool metabolites and microbiome composition were observed, providing valuable insights for studying transplant outcomes and interventions.

Plasma Microbial Cell-free DNA Next-generation Sequencing in the Diagnosis and Management of Febrile Neutropenia

Esther Benamu et al.

Summary: This study evaluated the ability of Karius microbial cell-free DNA sequencing test (KT) to identify infectious etiologies in patients with febrile neutropenia (FN) and its impact on antimicrobial treatment. The results showed that KT was able to detect organisms that were not identified by conventional microbiological tests, and it had the potential to improve the diagnosis and optimize the antimicrobial therapy for FN.

Short versus extended treatment with a carbapenem in patients with high-risk fever of unknown origin during neutropenia: a non-inferiority, open-label, multicentre, randomised trial

Nick A. de Jonge et al.

Summary: This study aimed to evaluate the safety of early antibiotic discontinuation in haematology patients undergoing chemotherapy-induced neutropenia. The results showed that stopping carbapenem treatment after three days did not increase treatment failure. However, the study also found that serious adverse events and all-cause mortality were more common in patients with persistent fever.

Blood transcriptomics identifies immune signatures indicative of infectious complications in childhood cancer patients with febrile neutropenia

Gabrielle M. Haeusler et al.

Summary: This study investigated the transcriptome of peripheral blood mononuclear cells (PBMCs) in children with cancer and febrile neutropenia (FN) to identify potential predictors of serious infection. The study found global differences in gene expression associated with specific immune responses in children with FN and documented infection, compared to episodes without documented infection. These differences may inform monitoring and aid in defining adequate treatment for different infectious aetiologies in children with cancer.

Summary: Early discontinuation of antimicrobials in pediatric patients with low risk febrile neutropenia in an outpatient setting is as effective as continuing therapy before recovery of counts. This approach can lead to significant cost and resource benefits.

Antibiotic Resistant Bloodstream Infections in Pediatric Patients Receiving Chemotherapy or Hematopoietic Stem Cell Transplant: Factors Associated with Development of Resistance, Intensive Care Admission and Mortality

Elio Castagnola et al.

Summary: Bloodstream infections (BSI) caused by resistant Gram-negatives in children receiving antineoplastic chemotherapy or HSCT have high occurrence rates, with previous exposure to antibiotics or colonization of resistant pathogens being possible causes. Resistant Gram-negatives are significantly associated with ICU admission and mortality, with treating centers playing an important role in the outcome of treatment. The evidence of significant center-level variations on AR, ICU admission, and mortality emphasizes the need for careful local antibiotic stewardship and infection control programs.

Early modifications of the gut microbiome in children with hepatic sinusoidal obstruction syndrome after hematopoietic stem cell transplantation

Riccardo Masetti et al.

Summary: This study found that a rich and diverse gut microbiome before hematopoietic stem cell transplantation was associated with a reduced likelihood of developing hepatic sinusoidal obstruction syndrome. Patients who did not develop the syndrome showed enrichment in health-associated commensals before the transplant. This high-diversity configuration may represent a common protective feature against complications of HSCT.

Broad-Spectrum Antibiotics and Risk of Graft-versus-Host Disease in Pediatric Patients Undergoing Transplantation for Acute Leukemia: Association of Carbapenem Use with the Risk of Acute Graft-versus-Host Disease

Caitlin W. Elgarten et al.

Summary: Carbapenems are associated with increased risk of acute graft-versus-host disease (aGVHD), especially with early exposure. Antipseudomonal penicillins show a trend towards increased risk, but with no statistical significance. There is no identified association between broad-spectrum cephalosporins and aGVHD. Further research is necessary to clarify the underlying mechanism of this association.
