Screening to Identify Postoperative Pain and Cross-Sectional Associations Between Factors Identified in This Process With Pain and Function, Three Months After Total Knee Replacement

Summary: This study described the screening and recruitment process of a randomized trial and evaluated the associations between knee pain and function 3 months after total knee replacement (TKR). The results showed that the severity of pain after TKR was associated with overall health, physical health, pain worry, and satisfaction with surgery outcomes. Functional limitation was associated with depression, pain worry, satisfaction with surgery outcomes, and pain acceptance.

Quality of meta-analyses of non-opioid, pharmacological, perioperative interventions for chronic postsurgical pain: a systematic review

Comparison of Sevoflurane and Propofol on the Incidence of Postoperative Pain and Quality of Life in Patients Undergoing Total Knee Arthroplasty With Chronic Pain Before Surgery

Summary: Sevoflurane anesthesia may have potential advantages in reducing postoperative pain in patients undergoing TKA with a preoperative VAS score > 4. At 3 days post-operation, fewer patients reported moderate pain and more patients reported no pain in the sevoflurane group compared to the propofol group. Similarly, at 3 months post-operation, more patients in the sevoflurane group reported no pain and fewer reported moderate pain compared to the propofol group.

Summary: The study evaluated the use of BMI cutoffs for determining eligibility for total knee arthroplasty (TKA) and found that relying solely on BMI as a criterion for surgery may deny patients the opportunity for important clinical improvements. Therefore, shared decision-making and predictive tools should be considered to balance the potential benefits of TKA for obese patients with the increased risks and costs associated with complications.

Chronic postsurgical pain after total knee arthroplasty: A prospective cohort study in Japanese population

Summary: This study aimed to elucidate the prevalence and risk factors of chronic postsurgical pain (CPSP) after primary total knee arthroplasty (TKA) in a Japanese population. The results showed that higher preoperative pain scores and postoperative coronal malalignment were independent risk factors for CPSP. Preoperative pain management may help reduce the incidence of CPSP in patients with severe pain and good coronal alignment after surgery.

Factors Associated With the Risk of Developing Moderate to Severe Acute Postoperative Pain After Primary Total Knee Arthroplasty: Results From the PAIN OUT Registry

Summary: The study identified chronic preoperative pain, general anesthesia, and the use of opioid analgesics as risk factors for moderate-severe acute postoperative pain (APOP) after total knee arthroplasty (TKA). No protective factors were observed. These findings suggest the need for new strategies in treating pain in TKA patients and further studies to identify acute pain predictors for better pain management in high-risk patients.

Collection and Reporting of Patient-reported Outcome Measures in Arthroplasty Registries: Multinational Survey and Recommendations

Summary: Results from 25 arthroplasty registries showed that most registries collected common PROMs, mainly including EuroQol-5 Dimension, Knee Injury, and Hip Disability. The majority of registries administered PROM questionnaires within 3 months before and 1 year after surgery. All registries collected patient demographics and clinical information such as age, sex, BMI, indications for arthroplasty, and comorbidity index.

Patient-reported outcomes after hip and knee arthroplasty RESULTS FROM A LARGE NATIONAL REGISTRY

Summary: This study aimed to describe the self-reported health and quality of life of patients undergoing elective total hip and knee arthroplasty in Australia. The results showed that there were significant improvements in pain, function, and overall health post-surgery, with the majority of patients satisfied with their procedure. However, approximately 10% of patients reported dissatisfaction with their surgery. Future analyses will focus on factors contributing to this dissatisfaction.

The use of patient-reported outcome measures in hip and knee arthroplasty in Alberta

At the micro level, PROMs data are utilised for individual patient and provider input, while at the meso level, continuous quality improvement reports are provided to healthcare providers to evaluate performance against key indicators. At the macro level, PROMs data are used to evaluate system performance and support health policy decision making across different jurisdictions or time periods.
