Device-detected atrial fibrillation in a large remote-monitored cohort: implications for anticoagulation and need for new pathways of service delivery

Relationship between device-detected burden and duration of atrial fibrillation and risk of ischemic stroke

Summary: Among patients with cardiovascular implantable electronic devices (CIEDs) and nonpermanent atrial fibrillation (AF) not receiving oral anticoagulation (OAC), the burden and duration of AF show a significant positive correlation to each other, but not with the CHA(2)DS(2)-VASc score. Despite the predictive value of CHA(2)DS(2)-VASc score for stroke/transient ischemic attack (TIA), the amount of AF (measured by either burden or duration) does not seem to have a significant impact on stroke risk in this population.

Summary: In individuals with stroke risk factors, ILR screening resulted in a three-times increase in atrial fibrillation detection and anticoagulation initiation but no significant reduction in the risk of stroke or systemic arterial embolism. These findings might imply that not all atrial fibrillation is worth screening for, and not all screen-detected atrial fibrillation merits anticoagulation.
