Imaging-defined necrosis after treatment with single-fraction stereotactic radiosurgery and immune checkpoint inhibitors and its potential association with improved outcomes in patients with brain metastases: an international multicenter study of 697 patients

Eric J. Lehrer et al.

Summary: This retrospective multicenter study investigated the predictors of treatment-related imaging changes (TRICs) and their impact on patient survival in the management of brain metastases using immune checkpoint inhibitors (ICIs) and stereotactic radiosurgery (SRS). The results showed that TRICs were associated with improved overall survival, indicating that the presence of TRICs may increase the survival time of patients after treatment with ICIs and SRS.

30-day mortality in patients treated for brain metastases: extracranial causes dominate

Carsten Nieder et al.

Summary: This study aimed to develop a model to predict the 30-day mortality risk in patients with brain metastases. The study found that extracranial tumor progression was the main cause of 30-day mortality, and there were few unexpected deaths. Logistic regression analysis identified 9 risk factors associated with 30-day mortality, and patients were categorized into different risk groups based on their point sum.

Radiation Therapy for Brain Metastases: An ASTRO Clinical Practice Guideline

Vinai Gondi et al.

Summary: This article presents the latest guidelines for the management of brain metastases, including advanced radiation therapy techniques and systemic therapies with central nervous system activity. The guidelines are based on a systematic review and provide recommendations using a predefined consensus-building methodology and grading system. Strong recommendations are made for the use of stereotactic radiosurgery (SRS) and upfront local therapy, with conditional recommendations for surgical resection and safe deferral of local therapy. Additional recommendations are made for patients with limited metastases and favorable prognosis.

Focal cavity radiotherapy after neurosurgical resection of brain metastases: sparing neurotoxicity without compromising locoregional control

Klaus-Henning Kahl et al.

Summary: In our single-center study, focal cavity radiotherapy was associated with high local control. In three out of four patients, whole-brain radiotherapy could be avoided in the complete course of disease, using radiosurgery as salvage approach without compromising neuro-control.

Summary: The study showed high incidence of brain metastases among patients with HER2+ and triple negative MBC, with lower incidence among patients with HR+/HER2- MBC. Further investigation is needed to assess the utility of brain metastases screening programs in these populations.

Cost-Effectiveness of Stereotactic Radiosurgery and Stereotactic Body Radiation Therapy in Treating Brain Metastases

Daphne B. Scarpelli et al.

Summary: Institutional analysis showed that single-fraction stereotactic radiosurgery (SRS) is a less expensive and time-saving option compared to stereotactic body radiation therapy (SBRT). Additional comparative analyses are necessary to explore the clinical and financial benefits of these radiation therapy techniques.

Radiotherapy for brain metastasis and long-term survival

Summary: The study found that radiotherapy for patients with brain metastases can improve the 1-year survival rate, especially for those with primary lung cancer. Stereotactic radiosurgery appears to be more effective than whole-brain radiotherapy in this context.

Risk of radiation necrosis after stereotactic radiosurgery for melanoma brain metastasis by anatomical location

Siujoon Choi et al.

Summary: This retrospective study explored anatomical factors leading to radiation necrosis (RN) in melanoma brain metastases (MBM) treated with stereotactic radiosurgery (SRS). Deep lesions in white matter structures were more likely to develop RN after SRS, with zone 2 lesions showing the highest incidence. Further studies with larger cohorts are needed to identify statistical differences in RN development rates in different anatomical zones.
