Summary: Background: The diagnostic and prognostic potential of trimethylamine N-oxide (TMAO) in functionally relevant coronary artery disease (fCAD) was investigated. Methods: Serum TMAO and its precursors were quantified in 1726 patients with suspected fCAD using liquid chromatography tandem mass spectrometry. The diagnosis of fCAD was performed using myocardial perfusion single photon emission tomography and coronary angiography. Results: TMAO, betaine, choline, and carnitine concentrations were significantly higher in patients with fCAD compared to those without, but had low diagnostic accuracy. TMAO was a strong predictor for incident death and cardiovascular disease (CVD) in patients with suspected fCAD. Conclusion: TMAO and its precursors were significantly associated with fCAD, but had limited diagnostic value. TMAO was an important predictor for death and CVD in patients with suspected fCAD.
