The role of antibody responses against glycans in bioprosthetic heart valve calcification and deterioration

Summary: In a large cohort of patients who underwent aortic valve replacement, antibody responses to glycans present in bioprosthetic heart valves, notably galactose-alpha 1,3-galactose and N-glycolylneuraminic acid, were implicated in valve calcification and deterioration. It was found that antibodies against xenoantigens galactose-alpha 1,3-galactose and N-glycolylneuraminic acid could mediate the deterioration of bioprosthetic heart valves through calcification. Results suggest that BHVs manufactured using donor tissues deficient in these antigens may have improved durability.

Summary: This study proposed a method of inducing graft remodeling through heparin coating in small-diameter decellularized arterial grafts, showing anti-thrombogenicity and endothelialization. Mechanical testing indicated no significant differences in strain characteristics and modulus among native tissues, decellularized scaffolds, and transplanted scaffolds. These findings suggest that heparin-immobilized ECM coating offers a functionalized biomaterial for developing small-diameter vascular grafts.

PE enhanced the mechanical properties of the pericardium, and the combination of DE + PE in an alternating cross-linking manner may be a promising option for developing bioprosthetic pericardia in the future.

Bioengineered percutaneous heart valves for transcatheter aortic valve replacement: a comparative evaluation of decellularised bovine and porcine pericardia

Summary: Glutaraldehyde-treated, surgical bioprosthetic heart valves and percutaneous heart valves (PHVs) often undergo structural degeneration within 10-15 years. Decellularised scaffolds of bovine and porcine pericardia were used to manufacture bioengineered PHVs, which demonstrated satisfactory hydrodynamic performance and resistance to crimping damage without leaflet alteration. The porcine tissue scaffold showed potential for long-term performance as a preferable option compared to bovine tissue in constructing advanced PHV replacements.
