Diagnostic imaging for suspected pulmonary embolism during pregnancy and postpartum: A comparative radiation dose study

Summary: This study compared the radiation dose exposure and diagnostic efficiency of CTPA and V/Q in pregnant and postpartum women suspected of PE. The results showed that CTPA had higher maternal and breast radiation dose compared to V/Q, but modern CT scanners achieve lower radiation doses. The fetal radiation dose was low. The diagnostic yield of the imaging modalities in pregnant and postpartum women is similar.

Construction of a digital fetus library for radiation dosimetry

Breast MRI during pregnancy and lactation: clinical challenges and technical advances

Summary: The breast undergoes significant changes in structure and function during pregnancy and lactation, affecting its imaging characteristics and potentially reducing the visibility of concurrent pathological processes. The high occurrence of benign gestational-related diseases further complicates the clinical and radiological evaluation of the breast during this period. This pictorial review highlights that, despite being currently underutilized, technical advancements and new clinical evidence support the use of unenhanced breast MRI during pregnancy and both unenhanced and dynamic-contrast enhanced imaging during lactation as effective supplementary modalities in the diagnosis of pregnancy-associated breast cancer (PABC).

A multidisciplinary team approach is crucial for imaging, clinical management, and treatment. Ultrasound and MRI are preferred imaging modalities, while radiation dose concerns should be addressed. Iodinated intravenous contrast material is safe, while gadolinium-based contrast material should be avoided. Whole-body MRI with diffusion-weighted sequences is ideal for screening and staging cancer in pregnancy.

The results indicated that the use of barium sulfate as an internal radioprotective shield produced counterproductive artifacts and tube current increments, limiting the evaluation of the lung and subsegmental lower lobe pulmonary arteries.
