Complications, Symptoms, Presurgical Predictors in Patients With Chronic Hypoparathyroidism: A Systematic Review

Summary: This study aimed to identify symptoms and complications associated with chronic hypoparathyroidism and evaluate the predictive value of early postoperative measurements of PTH and calcium. The results revealed nine common complications and symptoms likely associated with chronic hypoparathyroidism. PTH values above 10 pg/mL 12-24 hours after surgery were strong predictors of no chronic hypoparathyroidism development, while patients with PTH values below the threshold required careful monitoring.

Parameters of Vitamin D Metabolism in Patients with Hypoparathyroidism

Summary: This study evaluated the extended spectrum of vitamin D metabolites in patients with HypoPT compared to healthy individuals. The results showed that patients with HypoPT achieved an adequate level of 25(OH)D-3 with standard doses of cholecalciferol, but they had supraphysiological levels of 1,25(OH)(2)D-3 and increased production of inactive metabolite. The abnormality of vitamin D metabolism does not seem to interfere with the achievement of hypoparathyroidism compensation.

Etiology and Pathophysiology of Hypoparathyroidism: A Narrative Review

Summary: This article utilized a systematic review to investigate the etiologies and pathogenesis of hypoparathyroidism, with a particular focus on new knowledge about parathyroid gland viability and intraoperative parathyroid hormone monitoring. Non-surgical factors, especially genetic factors, play a significant role in hypoparathyroidism. Due to the widespread impact of PTH deficiency on multiple organ systems, the article provides a detailed review of skeletal, renal, neuromuscular, and ocular complications. The burden of illness on patients and their caregivers leads to reduced quality of life and increased social costs for this chronic endocrinopathy.

Hypoparathyroidism: update of guidelines from the 2022 International Task Force

Summary: The 2022 International Task Force guidelines for chronic hypoparathyroidism will be published soon, updating the original guidelines from 2016 with new information. Chronic postsurgical hypoparathyroidism is now defined as lasting for at least 12 months after surgery. Serum PTH <10 pg/mL in the first 12-24 hours after surgery may predict chronic postsurgical hypoparathyroidism. The guidelines also provide detailed information on the symptoms, complications, monitoring, and management of chronic hypoparathyroidism for the next five years.
