Frequency and Acceptance of Clinical Decision Support System-Generated STOPP/START Signals for Hospitalised Older Patients with Polypharmacy and Multimorbidity

Bastiaan T. G. M. Sallevelt et al.

Summary: The study aimed to evaluate the frequency and acceptance of CDSS-generated STOPP/START signals in hospitalized older patients with polypharmacy and multimorbidity. Results showed that 99% of patients had at least one signal generated, with an overall acceptance rate of 39%. Acceptance varied greatly among different signals, ranging from 2.5% to 75.8%. Various patient-related and setting-related factors influenced the acceptance of these signals.

Impact of pharmacist interventions on drug-related problems in general surgery patients: a randomised controlled trial

Salah AbuRuz et al.

Summary: This study demonstrates that drug-related problems are common among general surgery patients in Jordan, especially those related to drug safety and efficacy. Pharmacists' recommendations significantly contributed to resolving most of the identified drug-related problems and had a significant impact on improving medications used in general surgery patients.

Medication review interventions to reduce hospital readmissions in older people

Lauren Dautzenberg et al.

Summary: Medication review combined with medication reconciliation, patient education, professional education and transitional care was found to be more effective in reducing hospital readmissions compared to usual care, while medication review alone did not show a significant impact on readmissions. The quality of included studies raised some concerns and highlighted the need for higher quality trials in this area.

Summary: This study aims to reduce hospital readmissions in older adults through pharmacist-led peri-discharge interventions, using a pragmatic approach and randomized clinical trial design. The research will investigate the impact of various interventions on readmission rates, as well as barriers to and facilitators of implementing the PHARM-DC intervention.

Summary: The study aimed to analyze the effectiveness of clinical pharmacist services on drug-related problems and patient outcomes in inpatients with chronic kidney disease. Results showed that the pharmacist intervention group had a lower number of DRPs at discharge compared to the control group, indicating the significant role of hospital pharmacists in preventing DRPs and medication discrepancies in CKD inpatients.

Summary: This study found that hospital-based comprehensive medication reviews, including postdischarge follow-up, did not reduce the incidence of unplanned hospital visits in older patients. Alternative forms of medication reviews should be considered for improving health outcomes and tested in randomized clinical trials.

Optimizing Therapy to Prevent Avoidable Hospital Admissions in Multimorbid Older Adults (OPERAM): cluster randomised controlled trial

Summary: This study found that inappropriate prescribing was common in older adults with multimorbidity and polypharmacy admitted to hospital, and could be reduced through pharmacotherapy optimization intervention. However, this intervention did not have a significant effect on drug related hospital admissions, suggesting the need for additional efforts to improve patient outcomes.
