Survival following liver transplantation for locally advanced, unresectable intrahepatic cholangiocarcinoma

Summary: Intrahepatic cholangiocarcinoma (iCCA) has traditionally been considered a contraindication for liver transplantation (LT), but recent studies have shown positive outcomes for LT after neoadjuvant therapy. A center developed a protocol for neoadjuvant therapy and LT for locally advanced, unresectable iCCA patients in 2010. Patients who underwent LT had a high overall survival rate at 1, 3, and 5 years, with a higher than expected proportion of patients with genetic alterations in fibroblast growth factor receptor (FGFR) and DNA damage repair pathways. Further research is needed to determine criteria for LT in iCCA and factors predicting survival.

Intrahepatic cholangiocarcinoma: Is there a role for liver transplantation?

Summary: This study found that patients undergoing liver transplantation for intrahepatic cholangiocarcinoma demonstrate overall survival profiles similar to those undergoing surgical resection, and significantly better than patients treated with systemic therapy alone. Liver transplantation is an effective treatment modality in select patients presenting with localized intrahepatic cholangiocarcinoma.

Summary: This study analyzed the impact of era, tumor size, and neoadjuvant therapy strategies on the outcomes of liver transplantation for cholangiocarcinoma patients. The results showed that outcomes of LT for CCA seem to have improved over time, and multimodal neoadjuvant therapy is associated with improved survival in both iCCA and hCCA patients, regardless of tumor size.

Cumulative incidence of solid and hematological De novo malignancy after liver in a multicentre cohort

Summary: This multicenter cohort study analyzed the cumulative incidences of de novo malignancies after liver transplantation, revealing that non-cutaneous de novo malignancies had a higher incidence compared to Post-Transplant Lymphoproliferative Disorders (PTLD) and Solid Organ Tumors (SOT). The most frequently observed SOT were lung and head & neck tumors.

Further research is needed to establish internationally accepted expanded selection criteria for HCC patients seeking liver transplantation.
