Psychosis and Hopelessness Mediate the Relationship Between Reduced Sleep and Suicidal Ideation in Schizophrenia Spectrum Disorders

Summary: Suicide is a major cause of death among individuals with schizophrenia spectrum disorders (SSD). Sleep disturbances have been reliably linked to greater levels of suicidal ideation among individuals with SSD. The study found that individuals with disrupted sleep, positive symptoms, and/or hopelessness should be routinely screened for suicidal thinking.

Insomnia and suicide as reported adverse effects of second-generation antipsychotics and mood stabilizers

Summary: This study investigated the association between second-generation antipsychotics (SGAs) and mood stabilizers compared to clozapine and lithium, respectively, in terms of insomnia and suicide ideation/behavior/death (SIB). The results showed that other SGAs and mood stabilizers were significantly associated with increased reported odds of insomnia and SIB compared to clozapine and lithium. The findings support the antisuicidal properties of clozapine and lithium and suggest that beneficial effects on sleep may be one potential pathway underlying these properties.

Summary: Genetic evidence suggests a potentially independent and causal effect of insomnia on suicidal behavior, as it is significantly associated with risk of MDD, BP, and SB. Restless leg syndrome did not show a significant influence on these outcomes. Further clinical investigation on insomnia treatment for SB prevention or management is encouraged.

Insomnia, suicidal ideation, and psychopathology in Chinese patients with chronic schizophrenia

Summary: Insomnia is associated with suicidal ideation, lifetime suicide attempt, and higher psychopathology scores in inpatients with chronic schizophrenia. This study suggests that formal assessment of insomnia is relevant to the clinical care of patients with schizophrenia as an indicator of suicidal thinking and behavior, depression, and symptom severity.

Summary: This study revealed that physical neglect in childhood trauma is a strong and independent risk factor for suicidal ideation in patients with schizophrenia, with insomnia and physical neglect being risk factors for the severity of suicidal ideation. Insomnia was found to play a complete mediating role between physical neglect and suicidal ideation.

Terminal insomnia is significantly associated with current suicidal ideation, while initial/middle insomnia is significantly associated with a recent suicide attempt. Insomnia appears to be an important indicator in the clinical care of patients with schizophrenia.
