Abdominal normothermic regional perfusion in controlled donation after circulatory determination of death liver transplantation: Outcomes and risk factors for graft loss

Summary: The study shows the benefits of using A-NRP over SRR in cDCD liver transplants in Spain, with reduced risks of overall biliary complications, ischemic type biliary lesions, graft loss, and patient death. Cold ischemia time and re-transplantation indication are significant independent predictors for graft loss in cDCD livers with A-NRP. Further improvements are needed for cases with prolonged cold ischemia and technically complex recipients, suggesting a potential role for complementary ex situ perfusion preservation techniques.

National time trends in mortality and graft survival following liver transplantation from circulatory death or brainstem death donors

Summary: This study compared the mortality and graft failure rates between recipients of livers donated after circulatory or brainstem death. The results showed a significant decrease in mortality rates for DCD recipients, which eventually became similar to that of DBD recipients. However, there was no improvement in graft failure rates for either DCD or DBD recipients. These findings suggest that the use of DCD livers may be a viable option in regions with high waiting list mortality.

Solutions such as validated surrogate endpoints, collaborative studies, recognition of larger research projects, individual research areas, and fair funding structures are suggested to overcome these challenges.
