Abdominal normothermic regional perfusion in controlled donation after circulatory determination of death liver transplantation: Outcomes and risk factors for graft loss

Amelia J. Hessheimer et al.

Summary: The study shows the benefits of using A-NRP over SRR in cDCD liver transplants in Spain, with reduced risks of overall biliary complications, ischemic type biliary lesions, graft loss, and patient death. Cold ischemia time and re-transplantation indication are significant independent predictors for graft loss in cDCD livers with A-NRP. Further improvements are needed for cases with prolonged cold ischemia and technically complex recipients, suggesting a potential role for complementary ex situ perfusion preservation techniques.

National time trends in mortality and graft survival following liver transplantation from circulatory death or brainstem death donors

David Wallace et al.

Summary: This study compared the mortality and graft failure rates between recipients of livers donated after circulatory or brainstem death. The results showed a significant decrease in mortality rates for DCD recipients, which eventually became similar to that of DBD recipients. However, there was no improvement in graft failure rates for either DCD or DBD recipients. These findings suggest that the use of DCD livers may be a viable option in regions with high waiting list mortality.

Trials & Tribulations of Liver Transplantation- are trials now prohibitive without surrogate endpoints?

James Richards et al.

Summary: Liver transplantation has evolved from a pioneering stage to routine medical care over the past 5 decades. However, there are still significant challenges in addressing unmet needs and questions in clinical trials to improve patient outcomes. Solutions such as validated surrogate endpoints, collaborative studies, recognition of larger research projects, individual research areas, and fair funding structures are suggested to overcome these challenges.

Pilot study of a noninvasive real-time optical backscatter probe in liver transplantation

James A. Richards et al.

Summary: This study demonstrates the feasibility of real-time measurement of optical backscatter in donor livers, with early results indicating readings correlate with steatosis and may provide insights into graft outcomes such as early allograft dysfunction and graft loss.
