Risk scores in cardiac resynchronization therapy-A review of the literature

Summary: Cardiac resynchronization therapy (CRT) improves symptoms and reduces morbidity and mortality in selected heart failure (HF) patients, but the prognosis of HF remains poor. Tools are needed to assist in patient selection and provide prognostic information. We identified and provided the calculation methods and formulas for forty-eight risk scores in CRT. The reviewed score systems can predict CRT patients' prognosis, and some even have online calculation tools. However, the lack of cross-validation limits their routine use and objective selection.

Clinical implications of left atrial reverse remodelling after cardiac resynchronization therapy

Summary: The aim of this study was to evaluate the prognostic value of significant improvement in left atrial (LA) function and left ventricular (LV) function after initiation of cardiac resynchronization therapy (CRT). The results showed that patients with complete reverse remodeling had a lower risk of mortality compared to those with incomplete or no significant reverse remodeling. Integrated LA and LV deformation imaging may improve risk stratification of CRT recipients.

Left ventricle dyssynchrony may contribute to the onset and worsening of FMR, and is an important target for treatment.

Summary: This study aimed to investigate the relationship between anatomical and functional parameters measured by cardiovascular magnetic resonance (CMR) and the recurrence of paroxysmal atrial fibrillation (pAF) patients after catheter ablation. The study found that post-ablation minimum left atrial volume, expansion index, and baseline reservoir strain were associated with AF recurrence.

Exercise physiology of the left atrium: quantity and timing of contribution to cardiac output

Summary: This study explores the exercise behavior of the normal left atrium in response to graded effort by quantifying its volumetric changes. It found that low-level exercise decreases LA volume and increases ejection fraction, but these changes offset each other volumetrically. The major source of increased cardiac output with exercise is heart rate, while LA conduit fraction plays a key role in augmenting LV stroke volume.
