The 'Bermuda Triangle' of orthostatic hypotension, cognitive impairment and reduced mobility: prospective associations with falls and fractures in The Irish Longitudinal Study on Ageing

Desmond O. Donnell et al.

Summary: This study examines clustering of orthostatic hypotension, cognitive impairment, and mobility impairment, and their relationship with future falls and fractures in older adults. The cluster of these three syndromes is strongly associated with unexplained falls and incident fractures.

Unexplained falls in the elderly

Martina Rafanelli et al.

Summary: Falls in older adults are a complex issue that can lead to increased mortality, morbidity, and institutionalization. They are the leading cause of unintentional injury and are commonly seen in emergency departments. The causes of falls are multifactorial, involving individual susceptibility, high-risk activities, and environmental hazards. Evaluating the patient for various factors such as gait, balance, muscle weakness, and cognitive status is important, as well as assessing the residential environment for potential hazards. Not all falls can be explained, but the approach to managing unexplained falls is similar to that of unexplained syncope. Increasingly, the cardiovascular autonomic nervous system and implantable loop recorders are being used to investigate and diagnose falls.

Effect of chronic pain on the occurrence of falls in older adults with disabilities: a prospective cohort study

Hiroya Honda et al.

Summary: This study aimed to examine the relationship between chronic pain and falls in older adults with disabilities. The results showed that chronic pain significantly influenced the occurrence of falls. Therefore, it is important to consider the presence of chronic pain in falls prevention among older adults with disabilities.

A longitudinal study of the negative impact of falls on health, well-being, and survival in later life: the protective role of perceived control

Harpa Lind Jonsdottir et al.

Summary: This study examined the impact of falls on the health and well-being of older adults, finding that perceived control can mediate the negative effects of falls on health and well-being. The results suggest that older adults with higher levels of perceived control have better health and well-being outcomes after experiencing a fall.

Impact of a specialist service in the Emergency Department on admission, length of stay and readmission of patients presenting with falls, syncope and dizziness

K. Jusmanova et al.

Summary: The study demonstrates that establishing a dedicated falls and syncope service in the emergency department can significantly reduce admission rates and readmission rates for patients, as well as shorten the length of hospital stay, effectively guiding patients towards appropriate ambulatory care pathways.

STOPPFall(Screening Tool of Older Persons Prescriptions in older adults with high fall risk): a Delphi study by the EuGMS Task and Finish Group on Fall-Risk-Increasing Drugs

Lotta J. Seppala et al.

Summary: Healthcare professionals are often reluctant to deprescribe fall-risk-increasing drugs, leading to the development of STOPPFall and a deprescribing tool by a European expert group. Through an expert Delphi consensus process, 14 medication classes were included in STOPPFall, with practical deprescribing guidance developed for these classes. Evaluation of the effectiveness of these tools in falls prevention is recommended.

Anticholinergic burden measures and older people's falls risk: a systematic prognostic review

Carrie Stewart et al.

Summary: The studies found that moderate to high users of anticholinergic medications had a higher risk of falling, but it was less clear if lower burden users had an increased risk. Due to limited studies, it is currently unknown which scale is better than another.

E. P. van Poelgeest et al.

Summary: The use of antidepressants in older adults increases the risk of falls, but physicians are often reluctant to stop the prescription of these drugs. Practical resources and algorithms are available to guide and assist clinicians in this decision-making process.
