Fixed Airflow Obstruction in Asthma: A Problem of the Whole Lung Not of Just the Airways

Summary: Asthma with irreversible or fixed airflow obstruction (FAO) is a severe phenotype that is difficult to treat and is associated with accelerated decline in lung function and excess morbidity. Current understanding attributes FAO in asthma to airway remodeling driven by chronic inflammation, but emerging evidence suggests that changes in lung parenchyma and its elastic properties are equally or even more important in causing persistent lung function decline. This perspective highlights the need to consider structural and functional changes in parenchymal tissue when developing novel treatments for FAO in asthma.
