New advances in pharmacoresistant epilepsy towards precise management-from prognosis to treatments

Cenglin Xu et al.

Summary: This review summarizes the historical achievements and recent advances in the prognosis and treatment of pharmacoresistant epilepsy from both clinical and laboratory settings. It highlights the use of interdisciplinary techniques, such as biophysics, bioinformatics, biomaterials, and biochemistry, as well as the potential of novel experimental tools for precise treatment of pharmacoresistant epilepsy.

Activated astrocytes attenuate neocortical seizures in rodent models through driving Na+-K+-ATPase

Junli Zhao et al.

Summary: Research has shown that optogenetic stimulation of astrocytes through Na+-K+-ATPase can effectively attenuate seizures, providing a new approach and potential target for the treatment of intractable epilepsy.

Circuit Mechanisms Underlying Epileptogenesis in a Mouse Model of Focal Cortical Malformation

Weiguo Yang et al.

Summary: Our study revealed that in a mouse model, the occurrence of epilepsy is associated with specific synaptic reorganization within the cortical circuits, disrupting the balance between excitation and inhibition and leading to detrimental effects for the patients.

Focal to bilateral tonic-clonic seizures are associated with widespread network abnormality in temporal lobe epilepsy

Nishant Sinha et al.

Summary: The study found that in patients with TLE and FBTCS, the whole-brain structural network is altered to a greater extent and more widespread. These abnormal networks may serve as an underlying structural basis or consequence of the observed greater seizure spread in FBTCS.

Incomplete hippocampal inversion and epilepsy: A systematic review and meta-analysis

Carlotta Mutti et al.

Summary: Incomplete hippocampal inversion (IHI) is commonly observed in patients with malformation of cortical development (MCD), especially in periventricular nodular heterotopia or polymicrogyria. However, the presence of IHI is associated with only weakly increased odds of epilepsy development, indicating that it cannot be considered a strong independent predictor for epilepsy. More clear-cut neuroradiological criteria for IHI and advanced postprocessing analyses on MRI scans are recommended to differentiate between epileptogenic and nonepileptogenic IHI.

Histamine H1 receptor deletion in cholinergic neurons induces sensorimotor gating ability deficit and social impairments in mice

Li Cheng et al.

Summary: This study indicates that the deficiency of histamine H-1 receptor in cholinergic neurons in the basal forebrain is critical for sensorimotor gating deficit, social impairments, and anhedonia-like behavior in patients with schizophrenia. Deletion of the H1R gene in cholinergic neurons in mice resulted in functional deficiency of cholinergic projections and led to behavioral deficits, which could be rescued by re-expressing H1R or by chemogenetic activation of cholinergic neurons in the BF.

Subicular Caspase-1 Contributes to Pharmacoresistance in Temporal Lobe Epilepsy

Cenglin Xu et al.

Summary: The study demonstrated that activated caspase-1 in the subiculum contributes to pharmacoresistant temporal lobe epilepsy, and its inhibition can reduce neuronal excitability. Targeting the subicular caspase-1-interleukin-1 beta pathway may offer a novel alternative mechanism for treating pharmacoresistant TLE.
