The longitudinal dynamics and natural history of clonal haematopoiesis

Summary: Clonal haematopoiesis, a common phenomenon in human tissues during aging, was studied by tracking 697 clones from 385 individuals aged 55 years or older over a median of 13 years. The study found that 92.4% of the clones expanded at a stable exponential rate, with different mutations driving different growth rates. Clones with the same mutation had growth rates differing by approximately +/- 5% per year. The study also revealed that mutations driving faster clonal growth carried a higher risk of malignant progression.

Predictors of clonal evolution and myeloid neoplasia following immunosuppressive therapy in severe aplastic anemia

Summary: This study assessed predictors, genetic characteristics, and long-term outcomes of patients with severe aplastic anemia (SAA) who experienced clonal evolution after immunosuppressive therapy (IST). High-risk evolution was associated with older age and higher ANC levels. Patients with high-risk clonal evolution had lower overall survival rates, but those who underwent hematopoietic stem cell transplant (HSCT) showed improved survival. Certain gene mutations detected at 6 months after IST were predictive of high-risk evolution.

Clonality in immune aplastic anemia: Mechanisms of immune escape or malignant transformation

Summary: Aplastic anemia is a bone marrow failure syndrome that can be inherited or acquired. Inherited forms are caused by germline mutations, while acquired forms are thought to result from immune attack on hematopoietic stem cells. Chromosomal abnormalities and genetic variations have been found in aplastic anemia, indicating clonality. This review discusses the current concepts of clonality, somatic mutations, and their impact on diagnosis and clinical outcomes in immune aplastic anemia.
