Evaluation of Patient Reported Gastroesophageal Reflux Severity at Baseline and at 1-year After Bariatric Surgery

Anne P. Ehlers et al.

Summary: This study aimed to assess patient-reported GERD severity before and after SG and RYGB surgeries. The study found that although some SG patients experienced worsened symptoms after surgery, the majority of patients showed improvement or no change in GERD symptoms. Therefore, using patient-reported outcomes can help guide the choice of procedure in bariatric surgery for GERD patients.

Once in a Bile - the Incidence of Bile Reflux Post-Bariatric Surgery

Thomas A. Eldredge et al.

Summary: There is currently no "gold standard" for the investigation of esophageal bile reflux after obesity surgery, resulting in limited data on its incidence and severity and a lack of comparative studies. This study used specifically tailored biliary scintigraphy and upper gastrointestinal endoscopy protocols to evaluate esophageal bile reflux after OAGB, SG, and RYGB surgeries.

American Society for Metabolic and Bariatric Surgery 2020 estimate of metabolic and bariatric procedures performed in the United States

Benjamin Clapp et al.

Summary: There was a 22.5% decrease in the volume of metabolic and bariatric surgeries performed in the United States in 2020 compared to 2019. Sleeve gastrectomy remained the most common procedure, and single-anastomosis duodeno-ileostomy was reported for the first time in the MBSAQIP database.

Summary: After analyzing 10 studies, it was found that the prevalence of BE is high in patients undergoing EGD after SG, with no significant correlation with postoperative GERD symptoms. Most cases of BE were observed after 3 years.

High-resolution impedance manometry and 24-hour multichannel intraluminal impedance with pH testing before and after sleeve gastrectomy: de novo reflux in a prospective series

Tien Yew Chern et al.

Summary: The study found that laparoscopic sleeve gastrectomy (SG) significantly increased intragastric pressures, without affecting esophageal motility or acid exposure. For patients without preexisting reflux, there were significant increases in acid exposure time, long reflux episodes, weakly acidic reflux episodes, and DeMeester score after SG.

Manometric and pH-monitoring changes after laparoscopic sleeve gastrectomy: a systematic review

Andrea Balla et al.

Summary: This systematic review aimed to evaluate the changes in esophageal motility and acid exposure before and after laparoscopic sleeve gastrectomy (LSG) through esophageal manometry and 24-hours pH-monitoring. The results revealed a decrease in lower esophageal sphincter resting pressure and worsening DeMeester score and acid exposure time post-surgery, with a high rate of de novo GERD. Further long-term studies are needed to understand the clinical impact of LSG and the burden of GERD over time.

Indications and Long-Term Outcomes of Conversion of Sleeve Gastrectomy to Roux-en-Y Gastric Bypass

Antonio D'Urso et al.

Summary: Roux-en-Y gastric bypass after sleeve gastrectomy is a safe and effective revisional procedure to manage weight regain, de novo GERD, complications, and improve comorbidities. The percentage of excess weight loss at 1, 3, and 5 years post-bypass depends on the indication for revision.

Summary: This study investigated the safety, efficacy, and outcomes of conversion from SG to RYGB. It found that the conversion rate due to GERD was 30.4%, while due to ISWL/WR was 52.0%. The total body weight loss after 1 year was 22.8%.

GORD and Barrett's oesophagus after bariatric procedures: multicentre prospective study

Alfredo Genco et al.

Summary: This study investigated the prevalence of GORD and associated oesophageal complications following different bariatric procedures. The findings showed that SG had the highest prevalence of erosive oesophagitis and Barrett's oesophagus, as well as a higher rate of biliary-like gastric stagnation and biliary-type reflux into the oesophagus compared to other procedures.

Ramon Vilallonga et al.

Summary: This article discusses the importance of endoscopic diagnosis and follow-up for bariatric patients, particularly those with GERD symptoms. The study found that endoscopic follow-up after laparoscopic sleeve gastrectomy may be advisable even for asymptomatic patients due to pathological findings.
