Platelets in the neonate: Not just a small adult

Summary: Neonates, especially preterm infants, have a higher likelihood of decreased platelet counts and bleeding, with the belief that higher platelet transfusions could decrease the risk of bleeding. However, studies have found that neonates who receive more platelet transfusions have a higher incidence of bleeding and mortality. The mechanisms underlying the adverse effects of platelet transfusions in neonates are not clear, but it is hypothesized that transfusing adult platelets into a neonate's different physiological environment may result in developmental mismatch and negative consequences.

Hemostatic Challenges in Neonates

Summary: The neonatal hemostatic system differs significantly from that of adults, with newborns exhibiting hyporeactive platelets and decreased levels of coagulation factors, leading to a higher incidence of bleeding. However, the presence of other factors in neonatal blood effectively counteracts these deficiencies, resulting in a well-balanced hemostatic system slightly tilted toward a prothrombotic phenotype. Administering blood products to newborns based on laboratory tests may not decrease bleeding and could even increase neonatal mortality.
