Genomic landscape and clinical features of rare subtypes of pancreatic cancer: analysis with the national database of Japan

Summary: This study analyzed the clinical and genomic characteristics of patients with special subtypes of pancreatic cancer (ACC, ASC, ACP) and compared them with pancreatic ductal adenocarcinoma (PDAC) patients. It was found that ACC has different genomic features compared with PDAC, possibly accounting for differences in treatment efficacy.

SWI/SNF complexes in hematological malignancies: biological implications and therapeutic opportunities

Summary: Hematological malignancies are diverse diseases with varied molecular characteristics. SWI/SNF chromatin remodeling complexes are important for gene regulation in hematopoietic stem cells. Alterations in SWI/SNF complex subunits, particularly ARID1A/1B/2, SMARCA2/4, and BCL7A, are recurrent in lymphoid and myeloid malignancies. These alterations have clinical potential, as mutations in SWI/SNF subunits can confer resistance to antineoplastic agents and create synthetic lethality relationships with other proteins. SWI/SNF complexes and their subunits may be pharmacologically targeted for the treatment of hematological cancers.

Summary: Undifferentiated carcinoma of pancreas with osteoclast-like giant cells is a rare pancreatic tumor with difficulties in preoperative diagnosis. This study retrospectively analyzed the clinical data and treatments of 13 patients with this tumor. The results showed that elevated NSE levels and characteristic preoperative images can aid in the preoperative diagnosis. Surgical resection followed by chemotherapy is recommended to prolong survival.

Summary: Undifferentiated carcinoma (UC) and undifferentiated carcinoma with osteoclast-like giant cells (UCOGC) are peculiar variants of pancreatic ductal adenocarcinoma (PDAC), characterized by differences in inflammatory microenvironment, prognosis, and EMT activation. Snai2 emerged as a central EMT effector, with higher expression in UC compared to UCOGC, shedding new light on the biology of these entities.

Summary: Undifferentiated carcinoma with osteoclast-like giant cells of the pancreas is a rare and potentially aggressive variant of pancreatic ductal adenocarcinoma. Despite poor responses to immunotherapeutic approaches in the past, a patient with high tumor mutation burden and somatic mutations has shown remarkable response to pembrolizumab monotherapy after standard treatments failed. This case suggests the potential role of immunotherapy in treating rare cancers like UCOGC when standard treatments are not effective.

Risk stratification and prognostic factors in patients with unresectable undifferentiated carcinoma of the pancreas

This case report describes a patient with metastatic INI1-deficient undifferentiated rhabdoid carcinoma who showed primary resistance to FOLFIRINOX therapy but achieved a dramatic and sustained response to gemcitabine with nab-paclitaxel. More studies are needed to confirm these responses and potentially influence first-line therapy choices for this rare malignancy through multi-institution collaboration.
