Lower PbtO2 was associated with CPP values below calculated CPPopt. Induced hypertension results in cerebral perfusion pressures that exceed individual optimal cerebral perfusion pressure. Further studies are needed to explore the potential benefits of individual blood pressure management guided by optimal cerebral perfusion pressure based on autoregulation.

Cerebral Autoregulation Correlation With Outcomes and Spreading Depolarization in Aneurysmal Subarachnoid Hemorrhage

Summary: Delayed cerebral ischemia is one of the main therapeutic targets after aneurysmal subarachnoid hemorrhage. This study explores the correlation between different measures of cerebral autoregulation (CA), spreading depolarizations (SD), and neurological outcome in patients undergoing surgical treatment for aneurysmal subarachnoid hemorrhage. The results show that different continuous CA indices do not correlate well with each other on a highly time-resolved basis, and the blood pressure reactivity and oxygen reactivity are the most reliable indices in identifying the risk of worse outcome in these patients. The occurrence of SD is correlated with impaired CA and may represent the pathological mechanism of delayed cerebral ischemia.

Intraarterial Nimodipine Versus Induced Hypertension for Delayed Cerebral Ischemia: A Modified Treatment Protocol
