Summary: This study aimed to explore the treatment outcomes and prognostic factors for patients with previously irradiated but unresectable recurrent head and neck squamous cell carcinoma (rHNSCC) treated by stereotactic body radiotherapy (SBRT) plus cetuximab at a single institute in Taiwan. The results showed that the treatment response rate and survival rates were relatively low in this treatment approach, but the re-irradiation interval was found to be a significant prognostic factor for treatment response.

It is suggested that SBRT doses equivalent to 5-fraction doses of 40 to 50 Gy are used for retreatment.

High-level evidence is lacking, but pragmatic consensus guidelines based on expert opinions provide useful references for decision-making. Through iterative revisions, recommendations with high-to-moderate agreement were formulated regarding general treatment strategies and details of reirradiation.

Summary: This study analyzed the clinical data of 42 patients with head and neck cancer who received IMRT-based re-irradiation, showing that good patient performance, longer time intervals between radiotherapy courses, and higher re-irradiation doses were favorable factors for overall survival (OS). Meanwhile, higher re-irradiation doses and salvage surgery were significantly associated with improved progression-free survival (PFS). Further trials are required to identify patients who may benefit from re-irradiation without severe late toxicities.
