Health care transition from pediatric to adult care: an evidence-based guideline

Lars Pape et al.

Summary: A structured health care transition from pediatric medicine to adult medical care is necessary for young people with chronic health care needs. Germany has developed guidelines to support this transition, which include age-adapted patient education, involvement of caregivers, and a structured summary for the receiving physician. These guidelines may help establish globally accepted standards for appropriate support of young people.

Summary: The majority of children with congenital heart disease in high-income countries survive into adulthood, and paediatric cardiac services have expanded in middle-income countries, leading to an increasing number of survivors. Transitioning from dependent children to independent adults during adolescence, patients are advised to transfer to adult care without a universal consensus on how it should be provided. This consensus document addresses issues and practices for transition and transfer of adolescents with CHD across different income countries.
