Health care transition from pediatric to adult care: an evidence-based guideline

Germany has developed guidelines to support this transition, which include age-adapted patient education, involvement of caregivers, and a structured summary for the receiving physician. This consensus document addresses issues and practices for transition and transfer of adolescents with CHD across different income countries.
