The Optimization of Renal Graft Preservation Temperature to Mitigate Cold Ischemia-Reperfusion Injury in Kidney Transplantation

Summary: Renal transplantation is the preferred treatment for end-stage renal disease. The current gold standard for kidney preservation during transplantation is static cold storage at 4 degrees C. However, this method contributes to renal ischemia-reperfusion injury (IRI) which negatively affects graft survival and function. Recent studies focus on preserving kidneys at higher or subnormothermic temperatures to reduce the risk of cold renal IRI and enhance graft function and recipient survival.

The response to ESKD, particularly regarding KRT, depends on the local disease burden, culture, and socioeconomics. Disparities in access to KRT exist globally, with a treatment gap in LMICs demanding a focus on prevention strategies and affordable KRT options. Achieving global equity in KRT access will require concerted efforts in public policy, health care delivery, workforce capacity, education, research, and support from various organizations.
