Prevalence and Pathogenesis of Macro-Thyrotropin in Neonates: Analysis of Umbilical Cord Blood from 939 Neonates and Their Mothers

Summary: Macro-thyrotropin (macro-TSH) was found in 0.43% of neonates, and their mothers all had macro-TSH as well. It is recommended that if a baby's serum TSH concentration is high enough to consider congenital hypothyroidism but the free thyroxine level is normal, their mother's macro-TSH should be checked.

A Case of macro-TSH masquerading as subclinical hypothyroidism

Discontinuation of levothyroxine normalized free-T4 but TSH remained high, indicating a rare phenomenon known as macro-TSH. Recognition of macro-TSH can prevent misdiagnosis and inappropriate treatment.

Seasonal variations in levels of human thyroid-stimulating hormone and thyroid hormones: a meta-analysis

Summary: This study found that TSH levels are higher in winter and FT4 levels are higher in autumn. T3 levels are significantly higher in winter compared to summer, and FT3 levels are lower in summer compared to autumn and spring. The study also revealed gender differences in the seasonal dynamics of TSH and FT4, showing pronounced increases in TSH levels in winter for women and significant increases in FT4 levels in summer for men. Seasonal dynamics of TSH did not change with respect to age. The extent of the seasonal dynamics of TSH is influenced by the annual dynamics of oxygen density in the air, as well as meteorological factors such as atmospheric pressure and relative humidity.

Within-Person Variation in Serum Thyrotropin Concentrations: Main Sources, Potential Underlying Biological Mechanisms, and Clinical Implications

Summary: In euthyroid individuals, circadian rhythm has the greatest influence on fluctuations in serum TSH concentrations, with a nocturnal peak and daytime low. Seasonality also plays a role in TSH variations, with higher levels in winter. TSH levels fluctuate in minutes due to pulsatile secretion and increase with ageing. Other factors impacting TSH levels include TPO-antibody positivity, BMI, obesity, smoking, critical illness, and various xenobiotics.Potential underlying biological mechanisms of TSH variations include response to environmental cues for homeostasis. In clinical practice, considering within-person variations in TSH concentrations is crucial for accurate testing and research.

Summary: Inappropriate secretion of thyroid-stimulating hormone (IST) and syndrome of IST (SITSH) are clinical conditions characterized by elevated levels of free thyroxine and triiodothyronine along with detectable thyroid-stimulating hormone (TSH). While IST is associated with resistance to thyroid hormone beta caused by gene mutations, recent evidence suggests potential involvement of various factors such as hysteresis in the hypothalamus-pituitary-thyroid axis (HPT-axis), methodological interference, and postoperative Cushing's syndrome.
