Implications of enigmatic transglutaminase 2 (TG2) in cardiac diseases and therapeutic developments

Summary: Cardiac diseases are the leading cause of mortality and morbidity worldwide. Transglutaminases (TGs), particularly tissue TG (TG2), have been found to play important roles in the pathogenesis of cardiac diseases. TGs have various intra- and extracellular functions in the human body, including collagen cross-linking, angiogenesis, cell growth, and survival. TGs catalyze the formation of covalent bonds to increase the stability, rigidity, and stiffness of the myocardial extracellular matrix (ECM). Excessive accumulation of cross-linked collagen leads to myocardial stiffness and fibrosis. TG2, besides its role in extracellular protein cross-linking, may promote fibrotic diseases through cell survival and profibrotic pathway activation. TG2 has critical but incompletely understood roles in myocardial fibrosis and associated heart diseases, such as cardiac hypertrophy, heart failure, and age-related myocardial stiffness.
