2021 ELSO Adult and Pediatric Anticoagulation Guidelines

Ali B. McMichael et al.

Summary: These guidelines provide educational information for adult and pediatric anticoagulation in extracorporeal membrane oxygenation, aiming to support physicians and other health professionals in assessing and managing patients undergoing ECLS / ECMO. However, they are not consensus recommendations and do not guarantee successful outcomes. Adherence to guidelines should be coupled with clinical judgment and individual patient consultation.

Comparison of Bivalirudin Versus Unfractionated Heparin for Anticoagulation in Adult Patients on Extracorporeal Membrane Oxygenation

Erica A. Sheridan et al.

Summary: Extracorporeal membrane oxygenation (ECMO) patients require systemic anticoagulation to prevent coagulopathy. This study compared the safety and efficacy of bivalirudin and unfractionated heparin (UFH) in ECMO patients. The results showed similar rates of bleeding and thrombosis, but bivalirudin demonstrated better control of therapeutic range.

Bleeding and thrombotic events in adults supported with venovenous extracorporeal membrane oxygenation: an ELSO registry analysis

Jose Nunez et al.

Summary: This study aimed to analyze the prevalence, mortality association, and risk factors for bleeding and thrombosis events among adults supported with venovenous extracorporeal membrane oxygenation. Bleeding and thrombotic events are common during VV-ECMO and have a strong, cumulative association with in-hospital mortality. While thrombotic events are more frequent, bleeding carries a higher risk of inpatient mortality. Differential risk factors for bleeding and thrombotic complications exist, suggesting the possibility of a tailored approach to VV-ECMO management.

Unfractionated Heparin Versus Subcutaneous Nadroparin in Adults Supported With Venovenous Extracorporeal Membrane Oxygenation: a Retrospective, Multicenter Study

Pawel Piwowarczyk et al.

Summary: This study evaluated the safety and feasibility of using subcutaneous nadroparin compared with unfractionated heparin during respiratory ECMO. The results showed that nadroparin was equivalent to unfractionated heparin in terms of bleeding and thrombotic events, life-threatening bleeding incidence, and daily red blood cell transfusion rates during venovenus-ECMO.

Summary: The study reviewed thrombosis and bleeding outcomes in ECMO patients without continuous systemic anticoagulation. Results showed that while the incidence of bleeding and thrombosis was higher, the risk of circuity and patient thrombosis in anticoagulant-free ECMO patients was comparable to those receiving continuous systemic anticoagulation during ECMO.

Summary: Optimal anticoagulation monitoring is crucial in ECMO patients to prevent complications. This study evaluated the use of ROTEM compared to TEG and conventional coagulation tests in assessing hemostasis during ECMO, finding moderate correlation with standard tests for INTEM CT but not for Kaolin R time. Additional studies are needed to establish appropriate anticoagulation targets for ROTEM during ECMO.

Hemostasis, coagulation and thrombin in venoarterial and venovenous extracorporeal membrane oxygenation: the HECTIC study

Bruce Cartwright et al.

Summary: This study found significant differences in coagulation profiles and thrombin generation between patients supported with VA and VV ECMO, with VA patients showing a more pronounced consumptive coagulopathy and higher thrombin generation. VA patients required a lower heparin dose to suppress thrombin generation. Despite these differences, there were no significant variations in bleeding or thrombotic event rates between VV and VA patients when adjusted for days on ECMO support. More research is needed to develop tailored anticoagulation strategies based on the mode of ECMO support.

Summary: This study investigated the impact of a lower target ACT during ECMO on safety and outcomes, finding that it did not increase the thromboembolic risk compared to a conventional ACT target. Clinicians may consider using a lower ACT target for anticoagulation in some patients with careful assessment and close monitoring. Further prospective trials are needed to validate these results.

Argatroban versus heparin in patients without heparin-induced thrombocytopenia during venovenous extracorporeal membrane oxygenation: a propensity-score matched study

Summary: In patients without HIT on vvECMO, Argatroban was non-inferior to UFH regarding bleeding and thrombosis. The occurrence of technical complications was similarly distributed. Argatroban may have less impact on platelet decrease during ECMO, but this finding needs further evaluation. Direct drug costs were higher for Argatroban but comparable to UFH after accounting for HIT-testing and transfusions.
