Dabigatran etexilate for the treatment of acute venous thromboembolism in children (DIVERSITY): a randomised, controlled, open-label, phase 2b/3, non-inferiority trial

Summary: The study found that an age-adjusted and weight-adjusted dosing algorithm for dabigatran was effective in children with venous thromboembolism, showing similar pharmacokinetic-pharmacodynamic relationships to those seen in adults. In terms of efficacy, dabigatran was non-inferior to standard of care, indicating it may be a suitable alternative treatment option.

Summary: Plasma levels of markers of coagulation and inflammation have been identified as prognostic factors for adult postthrombotic syndrome (PTS), but in young patients following provoked deep venous thrombosis (DVT), low fibrinolytic capacity, cytokine levels, and veno-occlusion were identified as potential prognostic factors for development of PTS. Further investigation is needed to definitively evaluate these findings and identify additional potential prognostic factors.

Prevalence of thrombotic complications in children with SARS-CoV-2

Summary: Thrombotic complications in children with SARS-CoV-2 are rare, with most cases occurring in adolescent girls. D-dimer values are not specific enough to predict thrombotic complications, and consideration of adolescence and previous thrombotic risk factors may be necessary when initiating anticoagulant prophylaxis in pediatric patients with COVID-19.

Rate of thrombosis in children and adolescents hospitalized with COVID-19 or MIS-C

Summary: This study found that age >= 12 years, cancer, presence of a central venous catheter, and MIS-C were significantly associated with thrombosis in children hospitalized with COVID-19 or MIS-C. It is notable that the majority of thrombotic events occurred in patients aged 12 years and older, and nearly one-third of patients experienced thrombosis despite receiving thromboprophylaxis.
