Impact of electrochemotherapy in metastatic cutaneous melanoma: a contemporary systematic review and meta-analysis

Fausto Petrelli et al.

Summary: Electrochemotherapy with either intratumoral or intravenous bleomycin shows high therapeutic response in cutaneous metastatic melanoma, with no significant difference in efficacy based on different administration routes or tumor sizes. Some studies reported high local control rates and 1-year overall survival rates.

The efficacy of immune checkpoint blockade for melanoma in-transit with or without nodal metastases - A multicenter cohort study

Carl-Jacob Holmberg et al.

Summary: This study analyzed a large cohort of patients with melanoma in-transit metastasis (ITM) treated with immune checkpoint inhibitors (ICI) and found that ICI is an effective treatment option for ITM. The overall response rate was 56%, and the complete response rate was 36%. Future studies should evaluate the role of immunotherapy in multimodality therapy.

European consensus-based interdisciplinary guideline for melanoma. Part 1: Diagnostics: Update 2022

Claus Garbe et al.

Summary: This study provides recommendations for the diagnosis and treatment of cutaneous melanoma (CM) based on systematic literature reviews and the experts' experience. The study emphasizes the importance of clinical and dermatoscopic examination for the diagnosis of melanoma, followed by histopathological confirmation. Imaging diagnostics such as sequential digital dermatoscopy and full body photography are suggested for high-risk patients to improve early melanoma detection. The study also provides guidelines for classification, staging, and follow-up schemes, as well as the use of imaging examinations in different stages.

Health-related quality of life trajectories in melanoma patients after electrochemotherapy: real-world insights from the InspECT register

L. G. Campana et al.

Summary: This study evaluated the health-related quality of life (HRQoL) in melanoma patients before and after receiving electrochemotherapy (ECT) treatment. The results showed that HRQoL of complete responders was similar to the general population, but there was a temporary deterioration in pain/discomfort and mobility, as well as a sustained decline in self-care and usual activities. Combination with checkpoint inhibitors improved HRQoL outcomes. Baseline HRQoL could predict treatment response.

Electroporation and Immunotherapy-Unleashing the Abscopal Effect

Tobias Freyberg Justesen et al.

Summary: This article introduces the immune response induced by electroporation and the impact on the immune system, as well as discusses the results of clinical and preclinical trials investigating the combination of electroporation modalities and immunotherapy. The potential of combining electrochemotherapy and irreversible electroporation with immunotherapy in cancer treatment is promising and may change the current treatment approaches for tumors with low immunogenicity and high levels of immunosuppression.

Pembrolizumab versus placebo as adjuvant therapy in resected stage IIB or IIC melanoma (KEYNOTE-716): distant metastasis-free survival results of a multicentre double-blind, randomised, phase 3 trial

Georgina Long et al.

Summary: Adjuvant pembrolizumab is an efficacious treatment option for resected stage IIB and IIC melanoma, with a significant improvement in distant metastasis-free survival compared to placebo and continued reduction in the risk of recurrence. The adverse event profile of pembrolizumab is consistent with previous studies.

The efficacy of immunotherapy for in-transit metastases of melanoma: an analysis of randomized controlled trials

Roger Olofsson Bagge et al.

Summary: There is a lack of evidence from prospective clinical trials on the use of immunotherapy in patients with ITM. It is recommended to pool data from multiple institutions to examine the efficacy of available drug therapies in this patient population. More importantly, prospective clinical trials of locoregional treatments with or without systemic drug therapies are required.

Clinical characteristics and therapy response in unresectable melanoma patients stage IIIB-IIID with in- transit and satellite metastases

Anne Zaremba et al.

Summary: Cutaneous melanoma with in-transit metastases (ITM) remains a significant clinical challenge. This study retrospectively evaluated 191 patients with initially unresectable stage III ITM and satellite metastases, finding that patients without distant progression were more often female, older, and had a primary melanoma on the leg. Response to PD-1 inhibitors was lower than expected, indicating the need for further investigation into the biology of ITM development and immune system interactions.

Combination of Pembrolizumab with Electrochemotherapy in Cutaneous Metastases from Melanoma: A Comparative Retrospective Study from the InspECT and Slovenian Cancer Registry

Luca G. Campana et al.

Summary: An investigation into the effectiveness of Electrochemotherapy (ECT) in combination with pembrolizumab for cutaneous melanoma metastases showed that the combined therapy was safer and more efficacious in preventing further growth of metastases compared to pembrolizumab alone. Patients treated with the combined therapy had lower disease progression rates and longer survival. The study suggests that ECT may enhance the effects of pembrolizumab by acting as an in situ vaccination against cancer cells. Prospective studies are needed to validate these findings.
