Changes in the gut microbiota and risk of colonization by multidrug-resistant bacteria, infection, and death in critical care patients

Elisa Rubio Garcia et al.

Summary: This study aimed to investigate whether the diversity and composition of the intestinal microbiota determine the risk of multidrug-resistant organism (MDRO) acquisition, infection, and mortality in patients admitted to a liver intensive care unit (ICU). The results showed that higher bacterial diversity and abundance of certain bacterial taxa were associated with a lower risk of MDRO colonization, infection, and death. These findings suggest that microbial markers have prognostic value in critical care patients. Larger perspective studies are needed to further validate the significance of these markers in clinical practice.

Summary: Rectal colonization by MDROs is extremely frequent in critically ill patients with cirrhosis, and colonization increases the risk of infection by the colonizing resistant strain.

Current and future perspectives in the treatment of multidrug-resistant Gram-negative infections

Matteo Bassetti et al.

Summary: Microbial resistance is a serious threat to human health worldwide, especially with Gram-negative bacteria spreading resistance through mechanisms like AmpC enzymes, extended-spectrum beta-lactamases, and carbapenemases. Current treatments for multidrug-resistant Gram-negative bacteria involve a combination of older drugs and newer agents, with varying degrees of success. New treatment targets are also currently under investigation.
